Abstract
Objective To explore the value of vaginal ultrasound combined with serum carcinoembryonic antigen(CEA),alpha-fetoprotein(AFP),and anti-Müllerian hormone(AMH)in the evaluation of endometriosis(EMs).Methods A total of 108 EMs diagnosed and treated in our hospital from January 2022 to June 2024 were included.According to the American Society for Reproductive Medicine staging criteria,they were divided into a mild group(stages Ⅰ-Ⅱ,62 cases)and a severe group(stages Ⅲ-Ⅳ,46 cases).All patients underwent preoperative vaginal ultrasound and serum CEA,AFP,and AMH level measurements.Using postoperative pathological diagnosis as the gold standard,the diagnostic efficacy of vaginal ultrasound,CEA,AFP,and AMH(individually and combined)in assessing EMs severity was compared.Results Vaginal ultrasound di-agnosed 91 cases(positive rate:84.26%),including 50 cases(80.65%)in the mild group and 41 cases(89.13%)in the severe group.Preoperative serum CEA and AFP levels in the mild group were lower than those in the severe group(t=5.636,6.349;P<0.05),while AMH levels were higher(t=5.513;P<0.05).The combined detection accuracy(vaginal ultrasound+CEA+AF-P+AMH)for mild group and severe group was 93.55%and 93.48%,respectively,both higher than individual methods(χ2=24.092,19.026;P<0.05).ROC curve analysis showed the combined method had higher sensitivity than single indicators,with an AUC of 0.939,sensitivity of 89.13%,and specificity of 83.87%(P<0.05).Conclusion Compared with single-index detection,vaginal ultrasound combined with serum CEA,AFP,and AMH effectively diagnose EMs,demonstrates higher clinical value in assessing disease severity,and achieves better sensitivity and specificity.关键词
