3D-SWE与原发性乳腺淋巴瘤患者临床特征及预后关系
Relationship between 3D-SWE and Clinical Characteristics and Prognosis in Patients with Primary Breast Lymphoma

Abstract
Objective To explore the relationship between three-dimensional shear wave elastography(3D-SWE)and the clinical characteristics and prognosis of patients with primary breast lymphoma(PBL).Methods A total of 118 PBL patients admitted to Luzhou People's Hospital from May 2021 to May 2023 were selected as the research subjects.All 18 PBL patients were followed up for 1 year,and 8 patients were lost to follow-up.A total of 110 patients were divided into the sur-vival group(90 patients)and the death group(20 patients)according to different prognoses.All of whom received 3D-SWE.The quantitative parameters of 3D-SWE[Maximum elastic modulus(Emax),Elastic modulus standard deviation(Esd),Elastic modulus ratio between lesions and surrounding parenchyma(Eratio)]in PBL patients with different clinical characteristics were statistically analyzed.Spearman correlation analysis was used to evaluate the relationship between the aforementioned param-eters and clinical characteristics[Ann-Arbor staging,tumor diameter,lactate dehydrogenase(LDH)level].The survival status within 1 year of patients was used as the criterion for clinical prognosis.The general information and 3D-SWE quantitative parameters of patients with different prognoses were compared.The Cox regression equation was used to analyze the prognos-tic factors of PBL patients,and a receiver operating characteristic(ROC)curve was drawn to analyze the predictive efficacy of prognosis.Results There were significant differences in Emax,Esd,and Eratio among patients with different Ann-Arbor stag-ing,tumor diameter,and LDH levels(P<0.05).In PBL patients,Emax,Esd,and Eratio were positively correlated with Ann-Arbor staging(r=0.733,0.671,0.624),tumor diameter(r=0.618,0.562,0.593)and LDH(r=0.605,0.553,0.577)(all P<0.05).The 1-year survival rate of 118 patients with PBL was 81.82%and the mortality rate was 18.18%.The Emax,Esd,and Eratio values of the death group were higher than those of the survival group(P<0.05).After adjusting for LDH,tumor diameter and Ann-Arbor staging,Emax,Esd,and Eratio were identified as high risk factors for death in PBL patients(P<0.05).The combined prediction effi-cacy of Emax,Esd,nd Eratio for clinical prognosis in PBL patients was significantly superior to that of individual predictors.Conclusion Emax,Esd,and Eratio are positively correlated with Ann-Arbor staging,tumor diameter,and LDH in PBL patients.The combined application of these three factors has better predictive efficacy and net benefit,which can provide a favorable ref-erence information for clinical diagnosis and treatment.关键词
