脓毒症患儿血清NETs与5-HT的相关性及诊断意义
Correlation and Clinical Significance of Neutrophil Extracellular Traps and Serotonin in Pediatric Sepsis

Abstract
Objective To explore the expression levels and correlation of neutrophil extracellular traps(NETs)and se-rotonin(5-HT)in the serum of children with sepsis,as well as their predictive and diagnostic value for the disease.Methods A prospective study included 68 sepsis patients admitted to Jiangsu University Affiliated Hospital from November 2023 to December 2024 as the sepsis group,further divided into the general sepsis group(n=57)and the septic shock group(n=11)based on disease severity.Additionally,61 pediatric patients with non-infectious diseases hospitalized during the same period served as the con-trol group.Levels of NETs/cell-free DNA(NETs/cf-DNA),5-HT,tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and platelet(PLT)levels were compared between the patients in sepsis group and control groups,as well as children with dif-ferent conditions in sepsis.The correlation between cf-DNA,5-HT,TNF-α,and IL-6 in children with sepsis was observed through Pearson correlation analysis.ROC curve was used to analyze the diagnostic and predictive value of cf-DNA and 5-HT in pediatric sepsis.Results The concentrations of cf-DNA,5-HT,TNF-α and IL-6 were significantly elevated in pediatric patients with sepsis compared to the control group,whereas the PLT demonstrated a marked reduction relative to the control group(P<0.01).There were statistically significant differences in the levels of cf-DNA,5-HT,TNF-α,IL-6,and PLT among children with different conditions in the sepsis group(P<0.01).The septic shock group showed higher cf-DNA,5-HT,TNF-α,and IL-6 levels but lower PLT levels than the general sepsis group(P<0.01).Correlation analysis demonstrated that in the se-rum of pediatric patients with sepsis,cf-DNA,5-HT,TNF-α,and IL-6 exhibited significant positive correlations(P<0.01).ROC curve analysis showed that the area under the curve(AUC)of cf-DNA and 5-HT for children with sepsis were 0.901 and 0.795,respectively,and the cut-off values were 505.93 ng/mL and 2.04 μmol/L,respectively.Conclusion Sepsis in children is characterized by excessive NETs formation and 5-HT system activation,which correlate with inflammatory response severity and demonstrate diagnosis value.Monitoring cf-DNA and 5-HT levels may aid clinical assessment of sepsis severity.关键词
