Abstract
Objective This study aimed to investigate the blood glucose control status and potential influencing fac-tors in elderly patients with type 2 diabetes mellitus(T2DM)in the Wusong Community of Shanghai.Methods Elderly T2DM patients aged over 65 years who underwent health examinations at the Wusong Community Health Service Center in Baoshan District,Shanghai,from January 2022 to December 2024 were included.Participants were divided into a blood glucose control standard group(HbA1c<7%)and a non-control group(HbA1c≥7%)based on glycosylated hemoglobin(HbA1c)levels.After data cleaning,quality control,and analysis of clinical data,statistical methods were used to analyze differences in indicators between the two groups.Binary logistic regression was employed to identify influencing factors of blood glucose control and to establish a risk prediction model.Results A total of 3 891 elderly T2DM patients were included according to the inclusion and exclusion criteria,with 2 978 in the control standard group and 913 in the non-control group.Significant differences were ob-served between the two groups in BMI,waist circumference,hip circumference,systolic blood pressure,hemoglobin,mean cor-puscular volume(MCV),mean corpuscular hemoglobin concentration(MCHC),platelet hematocrit(PCT),platelet distribution width(PDW),neutrophil percentage,lymphocyte percentage,urine pH,urinary nitrite,urinary protein,urinary ketones,urinary occult blood,total bilirubin,triglycerides,high-density lipoprotein cholesterol(HDL-C),blood urea,serum uric acid,fasting plasma glucose(FPG),alanine aminotransferase(ALT),and cholesterol(all P<0.05).Further binary logistic regression analysis revealed that hemoglobin,MCV,total bilirubin,FPG,ALT,BMI,and serum uric acid were independent influencing factors of blood glucose control(P<0.05).A nomogram risk prediction model was constructed,with an AUC of 0.926(95%CI:0.906-0.945)by ROC analysis,indicating high accuracy.Internal validation of the model showed an accuracy of 0.889,and the calibration curve closely aligned with the Y-X line,demonstrating excellent predictive performance.Conclusion Logistic regression analysis identified hemoglobin,MCV,total bilirubin,FPG,ALT,BMI,and serum uric acid as independent factors influencing long-term blood glucose control in elderly T2DM patients.The nomogram risk model based on these factors effectively observe blood glucose control risk.关键词
