Predictive Value of Lipoprotein(a)Combined with the CatLet© Coronary Score for MACCE in AMI Patients Undergoing Emergency Percutaneous Coronary Intervention

Abstract
Objective To investigate the predictive value of lipoprotein(a)[LP(a)]combined with the CatLet© coro-nary score for major adverse cardiovascular and cerebrovascular events(MACCE)in acute myocardial infarction(AMI)patients treated with emergency percutaneous coronary intervention(PCI).Methods This retrospective study enrolled 307 consecutive acute myocardial infarction(AMI)patients who successfully underwent emergency PCI at the Department of Cardiology,Bozhou Hospital Affiliated to Anhui Medical University,from January 2020 to May 2022.Based on 1-year follow-up outcomes,patients were categorized into the MACCE group(n=78)and the non-MACCE group(n=229).Baseline lipoprotein(a)[LP(a)]levels were measured,and both CatLet© and Gensini scores were calculated for all participants.Multivariate logistic regression analysis was performed to identify independent predictors of MACCE,while receiver operating characteristic(ROC)curve analysis was employed to evaluate the predictive performance of LP(a),the CatLet© score,and their combination for MACCE.Results The MACCE group exhibited significantly higher LP(a)levels and CatLet© scores compared to the non-MACCE group(P<0.05).Mul-tivariate analysis identified both LP(a)(OR=2.293,95%CI:1.523-3.462,P=0.018)and the CatLet© score(OR=1.093,95%CI:1.032-1.164,P=0.003)as independent predictors of MACCE.ROC curve analysis demonstrated that the combination of LP(a)(-cut-off:169.68 pg/mL)and the CatLet© score(cut-off:18.6)significantly enhanced predictive performance(AUC=0.862),with a sensitivity of 88.46%and specificity of 83.33%.Delong test confirmed the superiority of the combined model over individual predictors(P=0.0089,Z=2.64 vs LP(a);P=0.034,Z=2.12 vs CatLet© score).Conclusion The combination of LP(a)and the CatLet© score provides a more accurate prediction of MACCE risk in AMI patients following emergency PCI compared to either predictor alone.This integrated approach offers a novel clinical strategy for prognostic evaluation.关键词
