脓毒症幸存者ICU获得性衰弱现状、影响因素及其对生活质量的影响
Current status and influencing factors of ICU acquired weakness in sepsis survivors,and its impacts for quality of life

Abstract
Objective To investigate the current status,influencing factors of intensive care unit(ICU)acquired weakness(ICUAW)in sepsis survivors,and its impacts for quality of life(QOL).Methods A total of 285 sepsis survivors treated in the ICU of the Second Affiliated Hospital of Hainan Medical University from January 2022 to January 2024 were prospectively selected.Muscle strength of sepsis survivors was assessed with the Medical Research Council(MRC)score at ICU discharge,and according to the presence of comorbid ICUAW,patients were divided into 204 cases in the non-ICUAW group and 81 cases in the ICUAW group.The 36-Item Short from Health Survey Scale(SF-36)was used to evaluate the QOL of sepsis survivors at the 3rd and 6th months after ICU discharge.The two groups of patients were compared in general information and clinical data,and multivariate logistic regression analysis was used to evaluate influencing factors for sepsis survivors experiencing ICUAW.Pearson correlation was performed to analyze the correlation between MRC score and SF-36 score of sepsis survivors,and multiple stepwise linear regression was performed to analyze the influencing factors for QOL of sepsis survivors.Results The incidence of ICUAW of sepsis survivors at ICU discharge was 28.42％(81/285).Multivariate logistic regression analysis showed that female(OR=1.812,95％CI:1.021-3.215,P=0.042),higher Charlson Comorbidity Index(CCI)score(OR=1.393,95％CI:1.129-1.719,P=0.002),higher sequential organ failure assessment(SOFA)score(OR=1.170,95％CI:1.048-1.307,P=0.005),and longer duration of mechanical ventilation(OR=1.144,95％CI:1.041-1.258,P=0.005)were independent risk factors for concurrent ICUAW in sepsis survivors.Pearson correlation analysis showed that the MRC score of sepsis survivors was significantly positively correlated with the SF-36 score at the 3rd and 6th months after ICU discharge,respectively(r=0.684,0.563,both P＜0.001).Multiple stepwise linear regression analysis showed that the MRC score was an influencing factor for QOL of sepsis survivors at the 3rd and 6th months after ICU discharge,respectively(both P＜0.05).Conclusion ICUAW is more common in sepsis survivors,mainly influenced by gender,CCI score,SOFA score and duration of mechanical ventilation,and ICUAW is closely related to QOL of sepsis survivors.关键词
