Abstract
Objective:To explore the application value of high-frequency ultrasound(HFU)combined with ultrasound elastography(UE)in the assessment of the nature of superficial lymph nodes.Method:A total of 82 patients with superficial lymph node enlargement who visited Qinghai Provincial Transportation Hospital from August 2022 to August 2024 were selected as the research subjects.All patients underwent HFU examination,UE examination and combined examination.Record HFU[the ratio of long diameter to short diameter(L/S),the maximum arterial blood flow velocity(Vmax),the minimum flow velocity(Vmin),and the resistance index(RI)in the portal area of the target lymph node]and UE parameters[the elastic ratio of lymph nodes to surrounding adipose tissue(SWE-ER),and the ratio of blue area].Based on the results of pathological tissue examination,the patients were divided into the benign group and the malignant group.The differences in the above parameters between the two groups were compared,and the receiver operating characteristic(ROC)curve was drawn to analyze the differential value of HFU and UE parameters alone and in combination for benign and malignant superficial lymph nodes.Result:Among the 82 patients,49 cases were diagnosed as benign and 33 cases as malignant through pathological examination.The accuracy rates of HFU and UE in differentiating benign and malignant superficial lymph nodes were 64.63%and 78.05%respectively.The diagnostic accuracy rate of the combined application of the two was 92.68%,and the consistency with pathological diagnosis was relatively high,with a Kappa value of 0.849.Compared with the benign group,the ratios of L/S,lymph node Vmax,RI,SWE-ER and UE blue area in the malignant group were higher,and the difference was statistically significant(P<0.05).The areas under the ROC curves(AUC)of L/S,lymph node Vmax,RI,SWE-ER,blue area ratio and the combination of the five for differentiating benign and malignant superficial lymph nodes were 0.739,0.733,0.819,0.737,0.757 and 0.947,respectively.Conclusion:HFU combined with UE can improve the differential diagnosis level of benign and malignant superficial lymph nodes and is worthy of clinical promotion.关键词
