The effect of BMP2 overexpression on tendon-bone healing in rabbit anterior cruciate ligament reconstruction using bone marrow mesenchymal stem cells

Abstract
Objective To investigate the role of overexpression of bone morphogenetic protein 2(BMP2)in ten-don-bone healing during anterior cruciate ligament(ACL)reconstruction in New Zealand rabbits,using bone marrow mesenchymal stem cells(BMSCs).Methods An intra-articular incision adjacent to the medial patella was performed to establish a rabbit model for ACL reconstruction.Seventy-two New Zealand white rabbits were randomly divided into four groups:sham-operation group,model group,BMSCNC group(implanted with BMSCs in fibrin glue infected with negative control vector),and BMSCBMP2 group(implanted with BMSCs in fibrin glue in-fected with BMP2-overexpressing lentivirus),with 18 rabbits in each group.Histological analysis(HE staining and Masson staining),Micro-CT,and biomechanical testing were used to evaluate tendon-bone healing,bone for-mation,and biomechanical properties after ACL reconstruction.Results In the BMSCNC and BMSCBMP2 groups,it could be seen that mature cartilage zones were gradually fused with adjacent tendons and bones.Tendon-bone tunnels with dense fibrous tissue and a small amount of osseous tissue closely connected were observed,along with prominent fibroblasts and collagen fibers.Compared to the sham-operation group,the model group exhibited se-vere damage to cartilage and bone structures,with decreased bone mineral density,bone volume fraction(BV/TV),and protein expression of alkaline phosphatase(ALP),osteocalcin(OCN),and osteopontin(OPN)at the tendon-bone interface(P<0.05).Additionally,the maximum load and stiffness of the tendon graft complex were reduced(P<0.05).In comparison to the model group,both the BMSCNC group and the BMSCBMP2 group showed improvement in cartilage and bone structure damage,with new bone formation observed around the graft.There was an increase in bone mineral density,BV/TV,and protein expression of ALP,OCN,and OPN at the tendon-bone interface,along with an elevation in the maximum load and stiffness of the tendon graft complex(P<0.05).Moreover,the BMSCBMP2 group demonstrated higher values than the BMSCNC group(P<0.05).Conclusion BMSCs with BMP2 overexpression can accelerate tendon-bone interface bone healing in the ACL reconstruction New Zealand rabbits model.关键词
