2021-2023年山东省烟台市HIV-1抗体不确定样本首次检测及随访结果分析

Abstract
Objective To explore the value of human immunodeficiency virus(HIV)antibody screening test re-sults,western blot(WB)band type and viral load(VL)in the diagnosis of HIV-1 antibody indeterminate sam-ples.Methods The subjects with antibody reactivity in screening tests[rapid test(RT)and enzyme-linked im-munosorbent assay(ELISA)]conducted in HIV confirmatory laboratory in Yantai of Shandong Province from 2021 to 2023 were collected for WB.A total of 72 subjects with HIV-1 indeterminate antibody were followed-up and HIV-1 VL test was performed.Results The detection rate of VL was 85.19%in 54 samples which were de-tected positive by both RT and ELISA.Statistical analysis revealed that the differences in the detection rate of VL among the four groups according to sample/cut-off(S/CO)value of 72 HIV-1 antibody uncertain samples were statistically significant(P<0.05).Among these groups,the detection rate of VL was highest when S/CO≥12.Gp160 p24(28/72)and gp160(23/72)had the most WB bands in the HIV-1 antibody indeterminate samples.The positive rate of VL among the cases with WB bands on env+pol,env+pol+gag,env+gag was 100%,100%and 93.94%,respectively.Five cases with VL>5 000 copies/ml was detected among the 19 cases lost to follow-up.Conclusion The VL test can accurately identify the infection status of individuals with HIV-1 indeter-minate antibody and shorten the diagnosis time of HIV infection.The results of screening test,S/CO and WB bands can provide a reference for the diagnosis of HIV-1 infection to a certain extent.关键词
