基于中气升降理论辨治发声性抽动障碍
Diagnosis and Treatment of Vocal Tic Disorder Based on the Theory of Ascending and Descending of Middle Qi

Abstract
Tic disorder is a common pediatric disease,and vocal tics are a subtype that poses diagnostic and therapeutic challenges.Traditional Chinese medicine(TCM)diagnosis and treatment demonstrate unique advantages in clinical practice.Plain Questions·Treatise on the Explanation of Pain proposes that"all diseases arise from qi disorder,"and the core pathogenesis of vocal tics lies in the dysfunction of qi ascending and descending.Middle qi serves as the pivotal axis for the movement of qi.Based on the theory of middle qi ascending and descending,this paper proposes that the core pathogenesis of vocal tics is the dysfunction of middle qi ascending and descending,with spleen-stomach dampness stagnation and impaired pivot movement as the initial triggers,leading to the pathological transmission of liver depression,lung stagnation,and heart-kidney disharmony.The key to the treatment lies in restoring the pivotal function of middle qi through a multidimensional approach,including resolving dampness and stagnation,strengthening the middle jiao,soothing the liver and dispersing lung stagnation,and clearing heart fire while nourishing kidney yin,thereby achieving harmonious movement of middle qi and balancing the four dimensions.This paper preliminarily explores a comprehensive diagnosis and treatment system combining syndrome differentiation and medication with TCM-specific behavioral therapies,aiming to provide clinical guidance.关键词
