基于虚瘀理论探索益真保心复方对模拟维持性血液透析心衰机制大鼠的心肌保护作用
Exploration of the Myocardial Protective Effect of Yizhen Baoxin Compound Formula on Rats with Simulated Maintenance Hemodialysis Heart Failure Mechanism Based on the Theory of Deficiency and Stasis

Abstract
Objective:To explore the myocardial protective effect of Yizhen Baoxin Compound Formula(YBCF)based on deficiency-stasis theory in a rat model simulating maintenance hemodialysis(MHD)-induced heart failure(HF)mechanism.Methods:A total of 106 SPF-grade SD rats were randomly divided into six groups:a blank group(n=6),modeling group(n=20),YBCF intervention Group 0(n=20,with YBCF formula only),YBCF Group 1(n=20,YBCF+Salvia miltiorrhiza15 g and Carthamus tinctorius10 g),Group2(n=20,YBCF+Salvia miltiorrhiza 30 g and Carthamus tinctorius 20 g),Group 3(n=20,YBCF+Salvia miltiorrhiza 60 g and Carthamus tinctorius 40 g),with equal males and females.The blank group received edible oil via gavage.The other groups were modeled by gavage with adenine-edible oil solution,intraperitoneal injection of doxorubicin,and potassium bromate solution instead of drinking water to simulate MHD-HF.When serum creatinine(Cr)increased to 1.5 times the baseline at the end of the first week,YBCF intervention groups began receiving respective formulas via gavage once daily.General conditions(every weekend),Western medicine indicators[body weight(every weekend),food intake(every weekend),blood urea nitrogen(BUN,every weekend),Cr(every weekend),C-reactive protein(CRP,on the 6th weekend),blood pressure(on the 6th weekend),heart rate(HR,on the 6th weekend)],cardiac ultrasound indices[left ventricular ejection fraction(LVEF),fractional shortening(FS),left ventricular end-diastolic diameter(LVEDd),left ventricular end-diastolic volume(LVEDV),on the 6th weekend],myocardial pathology(on the 6th weekend),and TCM syndromes[syndrome quantification scores(weekly),grip strength test and treadmill experiment(on the 6th weekend)]were observed and recorded.Results:(1)General conditions:The blank group exhibited good mental state,alertness,glossy fur,normal excreta,red moist tongue,and normal claws.The modeling and YBCF intervention groups gradually developed lethargy,dullness,dull fur,loose stools,dark red swollen tongue,blood stasis at nail beds,and abdominal distension with hair loss.The modeling group showed the most severe symptoms,while YBCF groups demonstrated gradual improvement compared with modeling group on every aspect from week 3 onward.(2)Western medicine indicators:The modeling group showed significant increases in uremic toxins,microinflammatory state,and cardiac preload(P<0.01),with significant decreases in left ventricular systolic function(LVEF,FS),blood pressure,alongside reduced body weight and food intake(P<0.01).YBCF interventions led to near-linear improvement(approaching blank group levels)with eventual divergence,with Group 1 showing optimal efficacy.Myocardial pathology:The modeling group exhibited severe pathological damage under light/electron microscopy.YBCF groups showed varying degrees of improvement—subtle under light microscopy but clearer ultrastructural improvements under electron microscopy.Overall improvement ranked as Group 3>Group 2>Group 1>Group 0,though Groups 2-3 showed declines in other efficacy indicators,making Group 1 optimal.(3)TCM syndromes:The modeling group had significantly elevated syndrome scores(P<0.01)and reduced physical endurance(decreased grip strength,increased electric shocks in treadmill tests)(P<0.05,P<0.01).YBCF interventions significantly improved these indices,with Group 1 performing best.Regarding Salvia/Carthamus dosage:absence of these herbs yielded poor efficacy;high doses enhanced myocardial pathology but worsened other indicators,making Group 1 optimal.Conclusion:YBCF protects myocardial tissue in MHD-HF model rats based on deficiency-stasis theory,restoring cardiac function.Solely addressing deficiency or excessive stasis resolution is inadvisable.关键词
