内热针短刺法治疗腰椎间盘突出症的临床观察
Clinical Observation of Short-Thrust Needling with Inner Heating Needle in Treating Lumbar Disc Herniation

Abstract
Objective:To observe the clinical efficacy of short-thrust needling with inner heating needle in treating lumbar disc herniation(LDH)and its effects on serum IL-1β and TNF-α levels.Methods:A total of 52 LDH patients treated in the Rehabilitation Qutpatient Department of the First Affiliated Hospital of Guangxi University of Chinese Medicine from May 2023 to December 2024 were selected as the study subjects.The patients were randomly divided into a filiform needle acupuncture group(26 cases,1 dropout)and an inner heating needle short-thrust group(26 cases,1 dropout).The filiform needle acupuncture group received acupuncture at Shenshu(BL23),lumbar Jiaji points,Weizhong(BL40),and Ashi points,5 times per week,with 1 week as a treatment course.The inner heating needle short-thrust group received short-thrust needling at bilateral Huatuo Jiaji points from L2 to S1 as the main points,along with needling at tender points or lesion sites in the lumbar and sacral soft tissues,once every 5 days,with 5 days as a treatment course.Both groups were treated for 3 courses.The changes in VAS scores,JOA scores,LFR scores,and serum IL-1β and TNF-α levels before and after treatment were compared,and the effects of the two groups on pain,lumbar flexion range,and serum inflammatory factors were observed.Results:After 3 courses of treatment,both groups showed significant efficacy.The total effective rate in the inner heating needle short-thrust group was 88%(22/25),higher than that in the filiform needle acupuncture group(76%,19/25),with a statistically significant difference(P<0.05).After treatment,the VAS scores of both groups decreased(P<0.05),with a greater reduction in the inner heating needle short-thrust group than in the filiform needle acupuncture group(P<0.05).The JOA scores and LFR scores of both groups increased(P<0.05),with a greater improvement in the inner heating needle short-thrust group than in the filiform needle acupuncture group(P<0.05).The serum IL-1β and TNF-α levels decreased in both groups(P<0.05),with a greater reduction in the inner heating needle short-thrust group than in the filiform needle acupuncture group(P<0.05).No serious adverse reactions occurred in either group.Conclusion:Short-thrust needling with inner heating needle has significant anti-inflammatory and analgesic effects in treating lumbar disc herniation and can improve the lumbar flexion range of motion in patients.关键词
