辛开苦降法辨治胃食管反流病思路探析
Analytical Study on the Treatment of Gastroesophageal Reflux Disease Using the Pungent-Dispersing and Bitter-Descending Method

Abstract
Gastroesophageal reflux disease(GERD)is a common digestive system disorder characterized by typical symptoms of reflux and heartburn.The pathogenesis of this condition is complex,with prolonged disease course and considerable treatment difficulty.The Pungent-Dispersing and Bitter-Descending method is based on the theory of medicinal properties in Traditional Chinese Medicine(TCM),employing combinations of pungent-warm and bitter-cold herbs to restore normal ascending-descending functions of the spleen and stomach and rebalance qi movement.This approach is widely applied in disorders characterized by cold-heat complex,dysfunction of ascending-descending,and pathogen intermingling.GERD primarily affects the esophagus and stomach,closely relating to the liver and spleen.Its core pathogenesis involves stomach failing to descend normally with rebellious upward movement of stomach qi,resulting in spleen-stomach dysfunction,qi stagnation,upward heat and downward cold,forming a cold-heat complex syndrome.Clinically,the cold-heat complex represents the key pathological mechanism.In clinical practice,the Pungent-Dispersing and Bitter-Descending method should be employed to address three pathological characteristics:"disordered ascending-descending,dampness-heat intermingling,and cold-heat complex."This involves three combined therapeutic strategies:"simultaneous ascending-descending regulation to restore qi movement,coordinated bitter-pungent herbs to separate and eliminate dampness-heat,and concurrent cold-heat medication to balance yin-yang."These approaches collectively restore normal spleen-stomach movement,resolve dampness-heat,and harmonize yin-yang,thereby achieving the therapeutic goal of"eliminating pathogens while restoring normal function.".关键词
