肌骨超声引导下体外冲击波治疗联合肌力训练对肩袖损伤的临床疗效研究
Clinical Study of the Effect of Ultrasonic Guided Extracorporeal Shock Wave Therapy Combined with Muscle Strength Training on Rotator Cuff Injury

Abstract
Objective:To explore the effects of ultrasonic guided extracorporeal shock wave therapy combined with muscle strength training on rotator cuff injury.Methods:120 patients with rotator cuff injuries were selected and randomly divided into conventional treatment(CT)group,muscle strength training(MST)group,extracorporeal shock wave(ESWT)group,and combination(ESWT+MST)group,with 30 patients in each group.The CT group received conventional physical therapy,acupuncture,etc.On the basis of CT group,the other 3 groups received muscle strength training,ultrasonic guided extracorporeal shock wave therapy,and muscle strength training combined with extracorporeal shock wave therapy.Pain,shoulder muscle strength(supraspinatus,infraspinatus,teres minor,superior trapezius,inferior trapezius and anterior serratus),active range of motion(AROM)and shoulder joint dysfunc-tion were evaluated by visual analogue scale(VAS),surface electromyography,protractor and Constant-Murley shoulder joint func-tion score(CMS)before and after treatment.Results:After treatment,the pain,shoulder muscle strength,AROM and shoulder joint function of 4 groups were significantly improved,and the differences were statistically significant(P<0.05).Compared with the CT group,the VAS scores and root mean square(RMS)values of the superior trapezius muscle in the MST group,ESWT group,and combination group were significantly decreased after treatment,while the RMS values of the superior spinatus muscle,infraspinatus muscle,teres minor muscle,inferior trapezius muscle,and serratus anterior muscle,as well as the AROM in the abduction,internal rotation and external rotation directions,and CMS scores were significantly increased(P<0.05).The VAS score of the combined group was significantly lower than that of the MST group and ESWT group,while the RMS values of the rotator cuff muscle group,AROM,and CMS score were significantly higher than those of the MST group and ESWT group,and the differences were statisti-cally significant(P<0.05).Conclusion:Ultrasonic guided extracorporeal shock wave therapy combined with muscle strength training can effectively reduce the pain of patients with rotator cuff injury,enhance the muscle strength of rotator cuff muscle group,and im-prove the shoulder joint function of patients,which is expected to provide a new direction for conservative treatment of rotator cuff injury.关键词
