"患者-家属-医护"三方视角下造血干细胞移植术后患者照护需求的质性研究
The post-hematopoietic stem cell transplantation care needs from patient-family-healthcare provider triadic perspectives:a qualitative study

Abstract
Objective To understand the lived experiences of care needs in post-hematopoietic stem cell transplantation patients from triadic perspectives(patients,family caregivers,healthcare providers),establishing theoretical underpinnings for optimizing care delivery models.Methods Employing a phenomenological method,semi-structured interviews were con-ducted with purposively sampled participants from January 2021 to June 2024.The cohort comprised 19 post-hematopoietic stem cell transplantation patients,15 family caregivers,10 physicians and 10 nurses recruited from the hematology depart-ment of Hangzhou First People's Hospital.Interview transcripts underwent thematic analysis using Colaizzi's seven-step phe-nomenological method to extract substantive themes.Results The lived experiences of post-hematopoietic stem cell trans-plantation care needs converged into two primary thematic domains.Triad-consistent care imperatives included infection pre-vention/control,medication adherence,nutritional support,and psychosocial care.Perspective-dependent care priorities in-cluded clinician-patient communication discrepancies,long-term outcome expectations,and familial role reconfiguration.Conclusion Post-hematopoietic stem cell transplantation care needs are multifaceted and should be understood by integra-ting the distinct perspectives of triadic perspectives.Establishing a"complementary and synergistic"collaborative mechanism is essential.Future strategies should include developing multimodal health education tools to bridge terminology gaps,estab-lishing genomics-driven individualized follow-up pathways,promoting a"symbiotic-supportive"family model,and creating a tripartite support network integrating"medical,social,and economic"dimensions to achieve personalized whole-cycle care.关键词
