HbA1c、AIP变化与老年急性前循环脑梗死患者颈动脉斑块形成的相关性
Correlation between changes of HbA1c and AIP and carotid plaque formation in elderly patients with acute anterior circulation cerebral infarction

Abstract
Objective To investigate the correlation between glycocated hemoglobin(HbA1c),plasma arterio-sclerosis index(AIP)and carotid plaque formation in elderly patients with acute anterior circulation cerebral infarction.Methods A total of 125 elderly patients with acute anterior circulation cerebral infarction who received treatment in Beijing Daxing District People's Hospital from April 2019 to July 2023 were selected as the infarction group,and 125 healthy who participated in physical examination in the same hospital during the same period were selected as the control group.Carotid artery color ultrasound was performed on all subjects,carotid intima-media thickness was recorded,and laboratory indexes such as HbA1c and blood lipid mea-sured.AIP was calculated based on triglyceride and high-density lipoprotein cholesterol levels,and AIP(cAIP)was corrected.According to the presence or absence of carotid plaque,the patients were further divided into two subgroups:no plaque group(57 cases)and plaque group(68 cases).Pearson correlation analysis was used to analyze the correlation between HbA1c,cAIP and carotid intima-media thickness.Logistic regres-sion was used to analyze the risk factors of carotid plaque formation.Results HbA1c and cAIP in infarction group were significantly higher than those in the control group(P<0.05).HbA1c and cAIP in the non-plaque group were significantly lower than those in the plaque formation group(P<0.05).A significant positive corre-lation existed between HbA1c,cAIP and carotid intima-media thickness(r=0.653,0.766,P<0.05)in elderly patients with acute anterior circulation cerebral infarction.Univariate analysis showed that HbA1c,cAIP,hy-pertension history,triglyceride,low-density lipoprotein cholesterol and fasting blood glucose were all associ-ated with carotid plaque formation(P<0.05).Multivariate Logistic regression analysis showed that HbA1c,cAIP and low-density lipoprotein cholesterol were all risk factors for carotid artery plaque(P<0.05).Con-clusion The increase of HbA1c and AIP levels is a high risk factor to induce carotid plaque formation in elderly patients with acute anterior circulation cerebral infarction,and can be used as an early predictor of ca-rotid plaque formation.关键词
