离断胸背血管桥支对重建乳房的影响
Impact of division of thoracodorsal vascular bridge branches on breast reconstruction
摘要
Abstract
Objective To explore the application value of dividing the thoracodorsal vascular bridge branches during one-stage breast reconstruction with latissimus dorsi flap in breast cancer breast reconstruction.Methods A retrospective analysis was conducted on the clinical data of 80 breast cancer patients who under-went modified radical mastectomy with preservation of the nipple-areola complex combined with one-stage breast reconstruction with latissimus dorsi flap in The First People's Hospital of Baiyin City and The Third People's Hospital of Gansu Province from May 2020 to March 2023.According to different treatment methods of the thoracodorsal vascular bridge branches during the operation,the patients were divided into an experi-mental group(41 cases)and a control group(39 cases).The control group retained the thoracodorsal vascular bridge branches,while the experimental group divided 1-2 thoracodorsal vascular bridge branches.The opera-tion indicators,breast surface aesthetic scores,patient satisfaction,postoperative complications,upper limb mobility before and 6 months after surgery,and quality of life(Quality of Life Scale for Cancer Patients)were compared between the two groups.Results The operation time of the experimental group was slightly shorter than that of the control group(P<0.05).Compared with the control group(64.10%),the patient satisfaction of the experimental group(85.37%)was higher(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P<0.05).There was no significant difference in upper limb mobility between the two groups before and 6 months after surgery(P<0.05).The quality of life of the experimental group at 3 months and 6 months after surgery was significantly higher than that of the control group(P<0.05).Conclusions For breast cancer patients undergoing modified radical mastectomy with preser-vation of the nipple-areola complex combined with one-stage breast reconstruction with latissimus dorsi flap,dividing the thoracodorsal vascular bridge branches during the operation showed that the experimental group was non-inferior to the control group.Moreover,the experimental group could help shorten the operation time,improve breast aesthetics,patient satisfaction and quality of life,basically had no impact on upper limb mobility,and ensured safety,which is worthy of clinical promotion and application.关键词
