目的 通过建立兰州市7~12岁健康汉族儿童的肺通气功能指标预计方程以获得预计值.方法 2023年6月—2024年5月在兰州市5个行政区选取2 087名7~12岁健康汉族儿童,采用德国耶格Master Screen肺功能仪进行肺通气功能检查.记录并分析第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、一秒率(FEV1/FVC)、呼气峰值流量(PEF)、外推容积(VBE)等10项肺通气功能指标.以年龄、身高、体重为自变量,对各指标进行多元逐步回归分析,建立兰州市7~12岁健康汉族男、女童的肺通气功能指标预计方程.结果 7~<8、8~<9、9~<10、10~<11岁组男童FVC、FEV1高于女童,9~<10、10~<11、11~<12、12~<13岁组男童FEV1/FVC低于女童(P<0.05).除FEV1/FVC和VBE/FVC外,其他8项指标均与年龄、身高和体重呈正相关关系(P<0.001).PEF与FEV1呈正相关关系(P<0.001).在此基础上分别建立了男、女童的肺通气功能指标预计方程,按性别输入年龄、身高、体重可计算出相应预计值.结论 本研究建立的预计方程可通过性别、年龄、身高、体重计算出兰州地区7~12岁儿童肺通气功能预计值.

Objective To obtain predicted values by establishing prediction equations for pulmonary ventila-tion function indicators in healthy Han children aged 7-12 years in Lanzhou.Methods From June 2023 to May 2024,a total of 2 087 healthy Han children were recruited from five administrative districts in Lanzhou,and pulmonary ventilation function was conducted using the German Jaeger Master Screen spirometer.Ten pulmonary ventilation function indicators were recorded and analyzed,including forced expiratory volume in one second(FEV1),forced vital capacity(FVC),one-second rate(FEV1/FVC),peak expiratory flow(PEF)and volume of back extrapolation(VBE).Multiple stepwise regression analysis was performed with age,height,and weight as independent variables to establish the prediction equations for pulmonary ventilation function indicators in healthy Han boys and girls aged 7-12 yesrs in Lanzhou.Results For the 7-<8,8-<9,9-<10,10-<11 age groups,boys had higher FVC and FEV1 than girls,while boys aged 9-<10,10-<11,11-<12,12-<13 had lower FEV1/FVC than girls(P<0.05).All indicators,except FEV1/FVC and VBE/FVC,were correlated with age,height,and weight(P<0.001).There was a significant positive correlation between PEF and FEV1(P<0.001).Prediction equations for pulmonary ventilation function indicators were developed separately for boys and girls.These equations allowed for the calculation of corresponding predicted values by inputting age,height,and weight,stratified by gender.Conclusion The prediction equations established in this study can calculate the predicted values of pulmonary ventilation function for children aged 7-12 years in Lanzhou based on gender,age,height,and weight.
