开放楔形胫骨高位截骨术对髌股关节的疗效分析
Analysis of the efficacy of open wedge high tibial osteotomy on the patellofemoral joint

Abstract
Objective To explore the clinical efficacy of open wedge high tibial osteotomy(OWHTO)on patellofemoral joint function.Methods The clinical data of 114 patients(136 knees)who underwent medial open wedge high tibial osteotomy from July 2018 to August 2022 in First Affiliated Hospital of Shandong First Medical University were retrospectively analyzed.Of these patients,52 were male and 62 were female,with ages ranging from 42 to 64 years(mean 53.8 years).Preoperative and 2 d postoperative imaging indices were recorded:Hip-knee-ankle angle(HKA),medial proximal tibial angle(MPTA),relative position of the mechanical axis of weight bearing line ratio(WBLR),posterior tibial slope(PTS),Insall-Salvati index,Caton's index,osteotomy distance,and bracing angle.Preoperative and 24-month postoperative knee function scores were recorded:Hospital for Special Surgery(HSS),American Knee Society Score(KSS);patellofemoral joint function scores:Visual analogue scale(VAS),Kujala score.Preoperative and postoperative imaging indices and functional scores were compared separately.Results The follow-up period ranged from 24 to 36 months,with a mean of(30±5)months.The patients'postoperative knee and patellofemoral joint function scores were significantly improved compared with the preoperative ones.All the knee joint imaging indexes showed good improvement compared with the preoperative period,and the patellar height decreased and the tibial posterior tilt angle increased after surgery compared with the preoperative period,the difference was statistically significant(P<0.05).Conclusion All of the knee joint imaging parameters after OWHTO showed a good trend of improvement compared with the preoperative period,and played a good corrective role.Knee and patellofemoral joint functions of the patients were significantly improved after OWHTO,and OWHTO would not adversely affect the patellofemoral joint in the short term.However,OWHTO does lead to a decrease in patellar height and an increase in PTS.Nevertheless,these changes do not affect patellofemoral joint function and OWHTO is a safe and effective knee preservation treatment.关键词
