关节镜清理联合高选择性去神经化治疗膝骨关节炎的临床观察
Clinical observation of arthroscopic debridement combined with highly selective denervation in the treatment of knee osteoarthritis

Abstract
Objective To observe the clinical efficacy of arthroscopic debridement combined with highly selective denervation in the treatment of knee osteoarthritis.Methods The clinical data of 40 patients with mid-stage knee osteoarthritis who were treated with arthroscopic debridement combined with highly selective denervation(the combined group)in Wuhan Orthopaedics Hospital of Integrated Traditional Chinese and Western Medicine from June 2019 to December 2021 were retrospectively analyzed and compared with those of 40 patients with mid-stage knee osteoarthritis who were treated with knee arthroscopic cleaning(the clean-up group).Operative time and intraoperative bleeding were recorded,and range of motion,Lysholm scores,and VAS scores were observed in both groups preoperatively and at 1,6,and 12 months postoperatively,and HSS scores were observed at the final follow-up.Results All patients in both groups were followed-up for 12-16 months,with an average of(12.15±3.35)months.In terms of operation time,the combined group took slightly longer than the clean-up group,and the difference was not statistically significant(P>0.05).In terms of intraoperative bleeding,the combined group bled slightly more than the clean-up group,with no statistical significance(P>0.05).In terms of range of motion,both groups improved at 1,6,and 12 months after surgery compared with that before surgery,and the difference was statistically significant(P<0.05);the range of motion of the combined group was better than that of the clean-up group,and the difference was statistically significant(P<0.05).In terms of Lysholm scores,both groups were elevated at 1,6,and 12 months postoperatively compared with the preoperative period,with statistically significant differences(P<0.05);the combined group had significantly higher Lysholm scores than the clean-up group at 1,6,and 12 months postoperatively,with statistically significant differences(P<0.05).In terms of VAS scores,the scores of both groups at 1,6,and 12 months postoperatively were lower than the preoperative period,and the difference was statistically significant(P<0.05);the combined group had significantly lower VAS scores than the clean-up group at 1,6,and 12 months postoperatively,and the difference was statistically significant(P<0.05).In terms of HSS scores at the final follow-up,the combined group had significantly higher HSS scores than the clean-up group at the final follow-up,and the difference was statistically significant(P<0.05).Conclusion Mid-stage knee osteoarthritis treated by arthroscopic debridement combined with highly selective denervation can effectively relieve pain,improve knee joint function,delay or even avoid knee joint replacement,and has certain clinical promotion value.关键词
