膝关节镜联合PRP治疗Kellgren-Lawrence Ⅲ级膝骨关节炎疗效分析
Effect analysis of knee arthroscopy combined with PRP in the treatment of Kellgren-Lawrence Ⅲ grade knee osteoarthritis

Abstract
Objective To investigate whether knee arthroscopy combined with platelet-rich plasma(PRP)treatment is more effective than knee arthroscopy alone for treating Kellgren-Lawrence(KL)Ⅲ knee osteoarthritis.Methods A retrospective study was conducted on patients with KL-Ⅲ grade knee osteoarthritis who underwent arthroscopic knee surgery at the Second Affiliated Hospital of Xingtai Medical College between June 2020 and December 2023.Forty-eight patients were treated with arthroscopic knee joint cleanup+PRP(PRP group),and 51 patients were treated with arthroscopic knee joint cleanup+injection of normal saline(conventional group).Outcome measures included KOOS score,HKA and complications.Patients were followed up at 1,3,6 and 12 months after the operation,and then annually.Results Ultimately,45 patients were included in the PRP group,with an average follow-up time of(13.27±3.58)months,and 47 patients were included in the conventional group,with an average follow-up time of(13.61±2.87)months.At follow-up of 1,3,6,12 months after the operation,the KOOS pain,living ability and motor ability scores of the PRP group were all higher than those of the conventional group(P<0.05).At the 3,6 and 12 months follow-up,the KOOS-symptom score and quality of life score in the PRP group were all higher than those in the conventional group(P<0.05).Intra-group comparison revealed that the scores of KOOS in PRP group and the conventional group after the operation were all higher than those before operation(P<0.05).There was no significant difference in lower extremity HKA between and within the two groups before the operation and during the postoperative follow-up(P>0.05).No complications such as hematoma or infection occurred in either group.Conclusion The clinical effect of knee arthroscopy combined with PRP in the treatment of KL-Ⅲ grade knee osteoarthritis is significantly better than that of knee arthroscopy alone,which is worthy of clinical promotion.关键词
