多角度骨道预制穿刺法行经皮弯角椎体成形术治疗轻度骨质疏松性椎体压缩骨折的疗效分析

Abstract
Objective To compare the efficacy and safety of multi-angle bone channel pre-formation puncture versus conventional puncture in percutaneous curved vertebroplasty(PCVP)for mild osteoporotic vertebral compression fractures(OVCF).Methods A retrospective analysis was conducted on 115 patients with mild OVCF who underwent PCVP at the Rocket Army Characteristic Medical Center of PLA from October 2022 to April 2024.Of these patients,60 patients were treated with the multi-angle bone channel pre-formation puncture method,while 55 received the conventional puncture method.The two groups were compared in terms of operative time,bone cement volume,cement leakage rate,cement distribution grading,visual analog scale(VAS)score,Oswestry disability index(ODI),anterior vertebral height,and Cobb angle.Clinical outcomes were evaluated at the final follow-up using the modified MacNab criteria.Results All surgeries were successfully completed.Patients were followed up for an average of(8.19±2.05)months(range 6-12 months).The multi-angle bone channel pre-formation group had a longer operative time[(37.42±4.56)min]compared to the conventional puncture group[(35.55±5.24)min],the difference was statistically significant(P<0.05).The bone cement volume in the multi-angle group[(5.73±0.68)mL]was significantly higher than that in the conventional group[(4.42±0.58)mL],the difference was statistically significant(P<0.05);but there was no significant difference in leakage rates(P>0.05).The excellent rate of bone cement distribution grading in the multi-angle group(90.00%)was significantly higher than that in the conventional group(63.64%),the difference was statistically significant(P<0.05).Postoperatively,both groups showed significant improvements in VAS,ODI,anterior vertebral height,and Cobb angle(P<0.05).At the final follow-up,the incidence of vertebral height loss in the multi-angle group(6.67%)was lower than that in the conventional group(20.00%),and the excellent rate based on the MacNab criteria was significantly higher in the multi-angle group(91.67%)compared to the conventional group(72.73%),the difference was statistically significant(P<0.05).Conclusion Both the multi-angle bone channel pre-formation puncture method and the conventional puncture method are effective for treating mild OVCF in PCVP.The multi-angle method has advantages in terms of cement distribution,complication rates and clinical outcomes,although it takes longer to perform.Further validation of its long-term efficacy is warranted.关键词
