摘要

Abstract

Objective To analyze the correlation between the Systemic Immune-Inflammation Index(SII)and in-hospital outcomes in patients with acute myocardial infarction-related cardiogenic shock(AMI-CS).Methods A retrospective analysis was conducted on 592 AMI-CS patients admit-ted to Qilu Hospital of Shandong University from January 2017 to December 2023.Patients were di-vided into a death group(n=178 cases)and a survival group(n=414 cases)based on in-hospital outcomes.Baseline data,laboratory parameters,and treatment information were collected.SII was calculated as platelet count × neutrophil count/lymphocyte count.Univariate and multivariate logis-tic regression analyses were performed to assess the association between SII and in-hospital mortality.Receiver operating characteristic(ROC)curves were plotted to evaluate predictive performance.Re-sults The death group exhibited significantly higher SII levels compared to the survival group(1868.81vs.1136.67,P<0.001).Multivariate analysis identified SII as an independent predictor of in-hospital mortality in AMI-CS patients(OR=3.015,95%CI:1.977~4.596,P<0.001).Other independent predictors included age(OR=1.038),lactate level(OR=1.150),revascularization(OR=0.408),and platelet-to-lymphocyte ratio(PLR)(OR=0.997).ROC curve analysis demon-strated that SII had an AUC of 0.704(95%CI:0.663~0.745,P<0.001)for predicting in-hospi-tal mortality,outperforming PLR(AUC=0.553,95%CI:0.501~0.605,P=0.04).Conclusions SII is significantly associated with in-hospital mortality risk in AMI-CS patients and serves as an ef-fective inflammatory biomarker for prognosis assessment,with superior predictive value compared to the novel biomarker PLR.Monitoring SII may aid in early identification of high-risk patients and guide clinical interventions.
