复方苦参洗剂治疗围绝经期外阴阴道假丝酵母菌病的效果及对致病菌毒力的影响
Efficacy of Compound Kushen Lotion on perimenopausal vulvovaginal candidiasis and influence on virulence of pathogenic strain

Abstract
Objective It is to observe the efficacy of Compound Kushen Lotion in the treatment of perimenopausal vul-vovaginal candidiasis(VVC)and its influence on virulence factors of pathogenic strain.Methods One hundred and forty patients with VVC of syndrome of dampness-heat pouring downward treated in Xintai Hospital of Traditional Chinese Medi-cine from August 2022 to January 2024 were selected and divided into control group and observation group according to ran-dom number table method,with 70 cases in each group.The control group was treated with clotrimazole vaginal tablet,while the observation group was washed with Compound Kushen Lotion on the basis of treatment in the control group,both groups were continuously treated for one week.The scores of traditional Chinese medicine(TCM)syndrome,morphological indicators(density and diversity of flora,classification of Lactobacillus)and functional indicators(vaginal pH,biochemi-cal metabolic indexes)in vaginal microecological evaluation,Candida albicans(CA)virulence[activity of invasive en-zyme(secretory acid protease,extracellular phospholipase),hemolytic activity,cell surface hydrophobicity(CSH),adhe-sion(H value)]of the two groups were compared before treatment and in 2 weeks after drug withdrawal,and the clinical efficacy of the two groups were assessed.Results In 2 weeks after drug withdrawal,the scores of TCM syndrome of the two groups were significantly lower than those before treatment,the morphological and functional detection indicators for the va-ginal microecological evaluation were significantly improved compared with those before treatment,the vaginal microecology tended to be balanced,the number of strains isolated were obviously decreased,the positive rate of invasive enzyme,H val-ue,enzymatic activity of secreted acid proteases and extracellular phospholipases were significantly lower,while the hemo-lytic activity and CSH values were significantly higher than those before treatment,the improvements of the above indexes in the observation group were significantly better than those in the control group,the differences were all statistically signifi-cant(all P<0.05).The total effective rate of the observation group was 91.43%(64/70),which was significantly higher than that 74.29%(52/70)of the control group(P<0.05).Conclusion Compound Kushen Lotion could effectively allevi-ate the clinical symptoms,improve vaginal microecology,weaken the virulence of CA in patients with VVC of syndrome of dampness-heat pouring downward,and its efficacy is better than that of simple western medicine alone.关键词
