自体心包主动脉瓣重建术在主动脉瓣病变中的应用
Application of autopericardial aortic valve reconstruction in patients with aortic valve disease

Abstract
Objective To evaluate the clinical effect of autopericardial aortic valve reconstruction in the treatment of aortic valve disease.Methods The data of 32 patients who underwent autopericardial aortic valve reconstruction from September 2017 to April 2021 were retrospectively analyzed.They were divided into AS(aortic stenosis)group and AI(aortic insufficiency)group.The effect of the operation was evaluated by ultrasonography to observe the transvalvular pres-sure gradient,transvalvular flow velocity,and the area of aortic valve regurgitation,as well as left ventricular end-diastolic diameter.Results The results of postoperative echocardiography showed that,24 months after operation,there were statisti-cally significant differences in the transvalvular pressure gradient and transvalvular flow velocity of the aortic valve at the AS group while comparing with those before operation(P＜0.01).The difference of left ventricular end-diastolic diameter at 7 days and 6 months after operation was statistically significant(P＜0.01),and difference of left ventricular end-diastolic diam-eter at 12 months after operation and before operation was statistically significant(P＜0.05).However,the difference of left ventricular end-diastolic diameter at 24 months after operation and before operation was not statistically significant(P＞0.05).In the AI group,there was statistically significant difference in the area of aortic valve regurgitation at 24 months after operation and before operation(P＜0.01).In addition,the difference of left ventricular end-diastolic diameter of the AI group at 7 days,6 months and 12 months after operation was statistically significant(P＜0.01),and the difference of left ventricular end-diastolic diameter at 24 months after operation was not statistically significant(P＞0.05).Conclusion Autopericardial aortic valve reconstruction has a good hemodynamic effect in the clinical application of aortic valve disease,with promising short-to mid-term outcomes.关键词
