Quantitative imaging and prediction of cerebral perfusion recovery:a study based on multi-delay arterial spin labeling and collateral flow efficiency

Abstract
Objective:Based on multi-delay arterial spin labeling(Multi-PLD ASL)technology,we introduced the collateral flow efficiency(CFE)index for the first time.By integrating cerebral blood flow(CBF)recovery rate and arterial transit time(ATT)changes,a multivariable comprehensive model was developed to evaluate the efficacy of reperfusion and its prognostic value in patients with acute ischemic stroke(AIS).Methods:Eighty AIS patients were divided into successful reperfusion group(67 cases)and failed reperfusion group(13 cases).All underwent Multi-PLD ASL imaging at baseline(pre-treatment)and 48 h post-treatment to acquire CBF and ATT data.Patient-specific clinical variables,including age,gender,and comorbidities,were documented.Statistical analyses were performed to assess the predictive value of CBF recovery rate,ATT reduction,and CFE.Results:The successful reperfusion group exhibited significantly higher CBF recovery rates(82.5±10.2 vs 56.1±8.9,P<0.001)and greater ATT reduction(12.3 s(IQR 10.5~15.2)vs 7.1 s(IQR 5.0~8.0),P<0.01)compared to the failed group.CFE was strongly correlated with CBF recovery rate(r=0.68,P<0.001).A multivariable model incorporating CBF recovery rate,ATT re-duction,and CFE achieved an area under the ROC curve of 0.94(95%CI 0.91~0.96)and an area under the PR curve of 0.94,indicating excellent predictive performance.Feature importance analysis identified CBF recovery rate(42%)and CFE(33%)as the primary predictors of reperfusion success.Conclusion:The CFE-based model,integrating CBF recovery and ATT re-duction,provides an efficient,non-invasive tool for predicting AIS reperfusion success and prognosis.关键词
