Value of dynamic CTA combined with CTP on evaluating collateral circulation and prognosis in patients with acute ischemic stroke

Abstract
Objective:To analyze the value of dynamic CT angiography(CTA)combined with CT perfusion(CTP)on evalu-ating the prognosis in patients with acute ischemic stroke(AIS).Methods:Eighty-seven patients with AIS in the hospital were selected from September 2022 to April 2024 as the research subjects.The CTP parameters were compared between groups,and the correlation between CTA collateral circulation score and CTP parameters was analyzed.The patients were followed up until 3 months after treatment,and the modified Rankin(mRS)score was assessed.According to the mRS score,the patients were classified into good prognosis group(n=55)and poor prognosis group(n=32),and imaging results were compared.Multivari-ate Logistic regression analysis was used to analyze the influencing factors of prognosis in AIS patients.ROC curve was drawn to analyze the value of CTP parameters on prognosis in AIS patients.Results:The CTA collateral circulation score of 87 pa-tients was(2.74±1.35)points,with 0～2 points in 29 cases and 3～4 points in 58 cases.Spearman correlation analysis showed that collateral circulation score was positively correlated with rCBF and rCBV(P＜0.05),and was negatively correlated with rMTT and rTTP(P＜0.05).NIHSS score,collateral circulation status,core infarct volume,rCBF,rCBV,rMTT and VCBV＜30％were independent risk factors affecting the prognosis of AIS patients(P＜0.05).ROC curve results showed that the AUC of rCBF,rCBV,rMTT and VCBV＜30％for the prognosis of AIS patients were 0.948,0.910,0.959 and 0.956 respectively.Conclusion:Dy-namic CTA can quickly evaluate the collateral circulation of AIS patients.CTP related parameters have a certain correlation with collateral circulation,which can reflect the status of collateral circulation.Dynamic CTA combined with CTP provides a reference for the prognosis evaluation of AIS patients.关键词
