超声黏性成像联合剪切波弹性成像诊断桥本甲状腺炎的价值研究
Value of ultrasound viscosity imaging combined with shear wave elasticity in diagnosis of Hashimoto's thyroiditis

Abstract
Objective:To investigate the clinical application of ultrasound viscosity imaging(Ⅵ)and shear wave elasticity(SWE)in the diagnosis of Hashimoto's thyroiditis(HT).Methods:Sixty-one patients with HT and 61 healthy volunteers were included.The differences of Ⅵ and SWE quantitative indexes between Hashimoto group and control group were retrospectively analyzed,and the correlation between the quantitative parameters of Ⅵ and SWE and thyroid peroxidase antibody(TPOAb),thyroglobulin antibody(TgAb)were analyzed.The ROC curve was drawn,and the diagnostic efficacy of the two methods in the diagnosis of HT alone and in combination was compared.Results:There were statistically significant in the max elasticity(Emax),the mean elasticity(Emean),the min elasticity(Emin),the standard deviation elasticity(Esd),the max dispersion coefficient(Dmax),the mean dispersion coefficient(Dmean),the min dispersion coefficient(Dmin),the standard deviation dispersion coefficient(Dsd),the max viscosity coefficient(Vmax),the mean viscosity coefficient(Vmean),the min viscosity coefficient(Vmin)and the standard deviation vis-cosity coefficient(Vsd)between HT group and control group(P＜0.05).Emean,Esd,Dmean,Dmax,Dmin,Dsd,Vmean were positively correlat-ed with TgAb,and Dmean was the most correlated with TgAb(r=0.362,P＜0.001).Quantitative parameters of Ⅵ and SWE were positively correlated with TPOAb,and Emean was the most correlated with TPOAb(r=0.398,P＜0.001).Emean and Dmean coefficient had the highest diagnostic efficiency in the diagnosis of HT,with AUC were 0.804 and 0.854,respectively.The diagnostic ef-ficiency of Ⅵ and SWE combined diagnosis was higher than that of single diagnostic index,with the AUC of 0.873,and the sensitivity,specificity,positive value and negative predictive value of 73.80％,88.50％,86.54％and 77.14％,respectively.Conclusion:Both Ⅵ and SWE have higher diagnostic efficiency in the diagnosis of HT,and their combination can improve the accuracy of HT,which is expected to provide a new non-invasive diagnosis idea for the clinical diagnosis and treatment of HT.关键词
