经食道超声与经胸超声在心房颤动患者左心耳血栓诊断中的应用观察
Application of transesophageal echocardiography and transthoracic echocardiography in diagnosing left atrial appendage thrombosis in patients with atrial fibrillation

Abstract
Objective:To explore the application of transesophageal echocardiography(TEE)and transthoracic echocardiogra-phy(TTE)in the diagnosis of left atrial appendage thrombosis(LAAT)in patients with atrial fibrillation(AF).Methods:Eighty-four patients with AF in the hospital were selected as the study subjects.All patients were diagnosed by TEE and TTE.With TEE diagnosis as the gold standard,the consistency of TTE and TEE in diagnosing LAAT in patients with AF was analyzed,and the TTE and TEE indexes were compared between thrombosis group and non-thrombosis group.Results:TEE diagnosed 30 cases of positive LAAT and 54 cases of negative LAAT among 84 AF patients.TTE diagnosed 32 positive cases and 52 negative cases,and the diagnostic sensitivity,specificity,accuracy rate,and Kappa value were 93.33％,92.59％,92.86％and 0.847.LVPWT and LAD in thrombosis group were significantly higher than those in non-thrombosis group(P＜0.05),while LVEF was significantly lower than in non-thrombosis group(P＜0.05),but there were no significant differences in IVST,LVEDD and LVESD between two groups(P＞0.05).LAAEF,LAAFV and LAAEV were significantly lower in thrombosis group than those in non-thrombosis group(P＜0.05),but no statistical differences in LAAD and LAAO were shown between two groups(P＞0.05).Con-clusion:TTE has a high consistency with TEE in the diagnosis of LAAT in patients with AF,and can be used as a clinical non-invasive examination alternative for TEE for clinical screening of early AF patients.关键词
