超声衰减参数成像评估超重肥胖患者代谢相关脂肪性肝病的应用价值
Application of ultrasound-guided attenuation parameter to assess metabolic associated fatty liver disease in overweight and obese patients

Abstract
Objective:To investigate the application value of ultrasound-guided attenuation parameter(UGAP)in the assessment of metabolic associated fatty liver disease(MAFLD)in overweight and obese patients.Methods:A total of 328 overweight and obese patients from the First Affiliated Hospital of Henan University of Chinese Medicine from August 2023 to August 2024 were selected,including 225 in the MAFLD group and 103 in the simple overweight and obese group.During the same period,172 healthy people matched by gender and age were selected as the normal control group.The difference of attenuation coefficient(AC)among the groups was compared,the correlation between AC and clinical indicators was investigated by Pearson correlation analysis.The independent sample t-test and multiple Logistic regression analysis were used to screen MAFLD risk factors,construct prediction model and analyze diagnostic efficacy.Results:AC values of MAFLD group,overweight and obese group and normal control group were(0.73±0.08),(0.57±0.04)and(0.54±0.06)/(dB·cm·MHz),respectively,and the difference between groups was statistically significant(P<0.05).AC,BMI and visceral fat thickness(VFT)were the risk factors for predicting MAFLD in overweight and obese patients.The optimal cut-off values were AC=0.635/(dB·cm·MHz),BMI=27.58 kg/m2 and VFT=66.115 mm,respectively.The AUC were 0.993,0.792 and 0.708,respectively.The regression model for MAFLD prediction was:-138.482+185.444×AC+0.645×BMI+0.057×VFT,and AUC was 0.997,and the prediction efficiency was greater than that of single AC index and that of AC+BMI bivariate prediction model.The diagnostic sensitivity and specificity were 96.4%and 98.1%,respectively.Conclusion:UGAP can be used for clinical screening to evaluate the prevalence of MAFLD in overweight and obese patients and dynamically monitor the disease process.AC combined with BMI and VFT prediction model can improve the accuracy of UGAP assessment of MAFLD in overweight and obese patients.关键词
