星状神经节阻滞对老年女性患者术后神经认知功能障碍防治作用:一项前瞻性、随机对照研究
Preventive effect of stellate ganglion block on postoperative neurocognitive dysfunction in elderly female patients:a prospective randomized controlled study

Abstract
[Objective]Postoperative neurocognitive dysfunction(PNCD)is a common complication in elderly patients following surgery,significantly affecting postoperative recovery and long-term quality of life.This study aims to evaluate the preventive and therapeutic effects of stellate ganglion block(SGB)on PNCD in elderly female patients and explore its underlying mechanisms.[Methods]This prospective randomized controlled trial enrolled 60 elderly female patients undergoing pelvic floor surgery at Hunan Provincial Maternal and Child Health Care Hospital between March 2023 and May 2025.Participants were randomly assigned to either the control group(n=30)or the SGB group(n=30).The SGB group received ultrasound-guided unilateral stellate ganglion block one day before surgery and on the day of surgery,while the control group received no intervention.Postoperative cognitive function,sleep quality,pain levels,inflammatory response,and stress markers were assessed using the Mini-Mental State Examination(MMSE),Numerical Rating Scale(NRS),Visual Analog Scale(VAS),interleukin-6(IL-6),and cortisol levels.[Results]On postoperative day 7,MMSE scores were significantly higher in the SGB group compared with the control group(26.36±2.80 vs.24.73±3.27,P=0.042).Additionally,cortisol levels(424.00±32.83 nmol/L vs.456.90±34.36 nmol/L,P=0.001)and IL-6 levels(39.96±5.23 pg/mL vs.43.40±5.03 pg/mL,P=0.012)at 24 hours postoperatively were significantly lower in the SGB group.Furthermore,the SGB group exhibited superior sleep quality scores on postoperative day 3(6.33±0.67 vs.5.90±0.84,P=0.031).[Conclusion]Perioperative SGB administration may effectively reduce the incidence of PNCD in elderly female patients by modulating autonomic nervous function,improving sleep quality,and attenuating inflammatory responses and stress levels.关键词
