PAV+模式在机械通气困难撤机患者中的应用价值研究
Application value of PAV+mode in difficult weaning patients on mechanical ventilation

Abstract
[Objective]To study the application value of proportional assist ventilation with load-adjustable gain factors(PAV+)mode in difficult weaning patients on mechanical ventilation.[Methods]A total of 110 difficult weaning patients admitted to the ICU of Jiangsu Peixian People's Hospital from January 2023 to December 2023 were divided into two groups using a random number table method.The control group used continuous positive airway pressure(CPAP)/pressure support ventilation(PSV)mode,and the observation group used PAV+mode.Observation indicators included clinical indicators,respiratory parameters,blood gas indicators,diaphragm muscle function,and stress levels.[Results]After treatment,the observation group showed higher extubation success rate,peak expiratory flow(PEF),tidal volume(VT),partial pressure of oxygen in arterial blood(PaO2),arterial oxygen saturation(SaO2),oxygenation index(OI),diaphragmatic excursion(DE),diaphragm thickening fraction(DTF),superoxide dismutase(SOD),and glutathione peroxidase(GSH-Px)compared with the control group,with P<0.05.Mechanical ventilation time and malondialdehyde(MDA)were lower than those in the control group,with P<0.05.[Conclusion]PAV+mode is beneficial for the treatment progress of difficult weaning patients,improves the success rate of weaning,and helps to improve various respiratory parameters and blood gas indicators in patients.It also demonstrates superior performance in protecting and restoring diaphragm muscle function and reducing patient stress levels.关键词
