Effect of Tissue Cross-Linking Combined with Chaijiang Formula on Bleb Scarring in a Rabbit Model of Glaucoma Filtration Surgery

Abstract

OBJECTIVE To investigate the effect of tissue cross-linking combined with Chaijiang Formula on bleb scarring in a rabbit model of glaucoma filtration surgery(GFS).METHODS A total of 45 New Zealand white rabbits underwent trabeculectomy in the right eye to establish the GFS model.The rabbits were randomly divided into the following groups:model group(MG),tissue cross-linking group(TC),and three groups treated with tissue cross-linking combined with Chaijiang Formula at low(L-CJ),medium(M-CJ),and high(H-CJ)doses.Each group consisted of nine rabbits.Additionally,a control group(CG)received no surgical intervention.The TC,L-CJ,M-CJ,and H-CJ groups received tissue cross-linking treatment,while L-CJ,M-CJ,and H-CJ groups were administered Chaijiang Formula orally at doses of 5.5,11.0,and 22.0 mL/kg per day,respectively,for 4 weeks.The remaining groups received an equal volume of 0.9%sodium chloride solution.Intraocular pressure(IOP)was measured periodically,and anterior segment photography was used to observe the filtering bleb morphology.After treatment,eyeball samples were collected for histological examination using hematoxylin-eosin(HE)and Masson´s trichrome staining to assess filtering channel structure and fibroblast proliferation.Quantitative real-time PCR(qRT-PCR)and Western blotting were employed to measure the mRNA and protein expression levels of transforming growth factor-β1(TGF-β1)and α-smooth muscle actin(α-SMA).RESULTS(1)IOP:On postoperative day 14,IOP in the MG group was significantly lower than that in the CG group,the differences were statistically significant(all P<0.05).IOP in the TC,L-CJ,M-CJ,and H-CJ groups was significantly lower than in the MG group,and IOP in the M-CJ and H-CJ groups was further reduced compared to the TC group,the differences were statistically significant(all P<0.05).(2)Fibroblast proliferation:The number of fibroblasts in the filtering channels of the TC,L-CJ,M-CJ,and H-CJ groups was significantly lower than in the MG group,with all Chaijiang groups showing lower counts than the TC group,the differences were statistically significant(all P<0.05).(3)Collagen proliferation:Similar trends were observed for collagen fiber proliferation,the differences were statistically significant(all P<0.05).(4)mRNA expression:TGF-β1 and α-SMA mRNA expression in filtering bleb tissues of the MG group was higher than that in the CG group,and significantly reduced in the TC,L-CJ,M-CJ,and H-CJ groups(all P<0.05),with further reductions in the M-CJ and H-CJ groups compared to the TC group,the differences were statistically significant(all P<0.05).(5)Protein expression:Protein levels of TGF-β1 and α-SMA followed similar patterns as mRNA,the differences were statistically significant(all P<0.05).CONCLUSIONS Tissue cross-linking combined with medium and high doses of Chaijiang Formula significantly inhibits fibroblast and collagen fiber proliferation,suppresses TGF-β1 and α-SMA expression,reduces postoperative scarring,and maintains filtering bleb function,thus effectively lowering intraocular pressure in GFS model rabbits.
