揿针治疗脾虚肝旺型小儿目劄的临床研究
Clinical Study on Intradermal Needle Therapy for Pediatric Eye Twitching with Spleen Deficiency and Liver Hyperactivity Pattern

Abstract
OBJECTIVE To observe the clinical efficacy of intradermal needle therapy for pediatric eye twitching with spleen deficiency and liver hyperactivity pattern.METHODS A total of 94 children(188 eyes)diagnosed with eye twitching due to spleen deficiency and liver hyperactivity at Shanxi Eye Hospital from November 2023 to February 2024 were enrolled.Participants were randomly divided into three groups:Intradermal needle group(Group A),auricular acupoint bean embedding group(Group B),and control group(Group C),with 30 cases(60 eyes)in each group.Treatments included intradermal needles,auricular acupoint bean embedding,and sodium fluorescein eye drops,respectively,for 14 days,followed by a three-month follow-up.Yale Global Tic Severity Scale(YGTSS)scores(including twitching frequency,intensity,duration,and accompanying symptoms)were recorded before treatment,at seven days,and 14 days post-treatment.Clinical efficacy and recurrence rates were evaluated.RESULTS(1)YGTSS scores:No significant differences were observed in baseline scores among the three groups(P＞0.05).At seven days post-treatment,no statistically significant difference was observed in the eye blinking frequency score of Group C compared to baseline(P>0.05),while all other scores across the three groups at both seven and 14 days post-treatment were significantly lower than pre-treatment levels(all P<0.05).Significant differences emerged among the three groups at seven days in attack intensity scores,concomitant symptom scores,and total scores(all P<0.05),whereas no significant differences were found in eye blinking frequency scores or attack frequency scores(both P>0.05).At 14 days post-treatment,significant differences were observed among the three groups for all evaluated scores(P<0.05).At seven days post-treatment,Group A demonstrated significantly lower scores than Group C across all parameters(P<0.05),and Group B showed a significantly lower attack intensity scores than Group C(P<0.05),with no significant differences detected between Group A and Group B in any scores(all P>0.05).At 14 days post-treatment,all scores in Group A remained significantly lower than those in Group C(P<0.05),Group B exhibited a significantly lower concomitant symptom scores than Group C(P<0.05),and Group A additionally demonstrated a significantly lower total scores compared to Group B(P<0.05).(2)Clinical efficacy:The total effective rates were 93.33%(Group A),76.67%(Group B),and 63.33%(Group C),with significant differences(χ2=13.250,P=0.010).Group A outperformed Group C(χ2=13.290,P=0.001).There were no statistically significant differences when Group B was compared to either Group A or Group C(P>0.05).(3)Recurrence rates:Recurrence rates were 7.14%(Group A),17.39%(Group B),and 47.36%(Group C),with significant differences(χ2=6.250,P=0.044).Group A had a significantly lower recurrence rate than Group C(χ2=5.364,P=0.020).There were no statistically significant differences when Group B was compared to either Group A or Group C(P>0.05).(4)Safety:One case in Group A experienced transient pain due to overstimulation,which resolved after needle removal.No adverse reactions occurred in the other groups.CONCLUSIONS Intradermal needle therapy is safe,effective,and well-tolerated for pediatric eye twitching with spleen deficiency and liver hyperactivity pattern,warranting clinical promotion.关键词
