血清CysLTs、MMP-9对儿童腺病毒肺炎发展为闭塞性细支气管炎的预测价值
Predictive value of serum CysLTs and MMP-9 on the development of bronchiolitis obliterans in children with adenovirus pneu-monia

Abstract
Objective To explore the predictive value of serum cysteinyl leukotrienes(CysLTs)and matrix metallo-proteinase-9(MMP-9)on the development of bronchiolitis obliterans(BO)in children with adenovirus pneumonia(AP).Methods A retrospective analysis was performed on 127 AP children aged 1-5 years who were treated at Handan Maternal and Child Health Care Hospital from January 2021 to December 2023(AP group).According to the diagnostic criteria for severe pneumonia in the Standards for the Diagnosis and Treatment of Childhood Community-Acquired Pneumonia(2019 Edition),the children were divided into a non-severe group(52 cases)and a severe group(75 cases);according to the occurrence of BO,they were divided into a non-BO group(79 cases)and a BO group(48 cases).Another 65 healthy children who underwent physical examinations in the same hospital during the same period were selected as the control group.Enzyme-linked immunosorbent assay was used to detect serum CysLTs and MMP-9 levels,and clinical data of the children were collected.Serum CysLTs and MMP-9 levels were compared among groups.Multivariate logistic regression analysis was used to identify risk factors for BO development in AP children;receiver operating characteristic(ROC)curve was used to evaluate the predictive efficacy of serum CysLTs and MMP-9 on BO development in AP children.Results Serum CysLTs and MMP-9 levels in the AP group were both higher than those in the control group(both P<0.01).Serum CysLTs and MMP-9 levels in the severe group were both higher than those in the non-severe group(both P<0.01).The duration of fever in the BO group was longer than that in the non-BO group,and serum CysLTs and MMP-9 levels were both higher than those in the non-BO group(all P<0.05).Multivariate logistic regression analysis showed that long duration of fever,high serum CysLTs and MMP-9 levels were all risk factors for BO development in AP children(all P<0.01).ROC curve analysis showed that the area under the curve(AUC)of combined prediction of BO development in AP children by serum CysLTs and MMP-9 was 0.906,which was higher than the AUCs of single prediction by serum CysLTs or MMP-9(both P<0.01).Conclusion Serum CysLTs and MMP-9 levels are high in AP children.Long duration of fever and high serum CysLTs and MMP-9 levels are risk factors for BO development in AP children.Combined detection of serum CysLTs and MMP-9 has good predictive value for BO.关键词
