老年肌少症患者功能状态评估及发病影响因素研究
Assessment of functional status and influencing factors in elderly patients with sarcopenia

Abstract
Objective To evaluate the functional status of elderly patients with sarcopenia and explore the influencing factors of its incidence.Methods A multicenter cross-sectional study was conducted.By typical sampling,224 community-dwelling elderly patients aged 60 and above with chronic diseases were selected from Hangzhou,Quzhou,Jiaxing and other regions in Zhejiang Province from August 2024 to January 2025.Clinical data of the patients were collected,and sarcopenia assessment and related functional evaluations were carried out.Multivariate logistic regression was used to analyze the influencing factors of sarcopenia in the elderly patients.The ROC curve was applied to evaluate the efficacy of influencing factors in predicting sarcopenia of elderly patients.Results Among 224 elderly patients,60 were diagnosed with sarcopenia,with a prevalence of 26.8%.Univariate analysis showed that the proportions of patients aged ≥80 years,living alone,multiple medications,visual impairment,oral function decline,depression,malnutrition,cognitive function decline,frailty,and activity ability decline in the sarcopenia group were higher than those in the non-sarcopenia group,while body mass index(BMI),married status,and alcohol consumption proportions were lower than those in the non-sarcopenia group,with statistically significant differences(all P<0.05).Multivariate logistic regression analysis showed that high BMI(OR=0.831,95%CI:0.726-0.951,P=0.007)was a protective factor for sarcopenia in elderly patients.Age ≥80 years(OR=5.427,95%CI:1.862-15.819,P=0.002),multiple medications(OR=7.250,95%CI:1.934-27.172,P=0.003),and malnutrition(OR=3.131,95%CI:1.044-9.393,P=0.042)were risk factors for sarcopenia in elderly patients.ROC curve analysis showed that the predictive efficacy of sarcopenia-related influencing factors was ranked as:age>BMI>multiple medications>malnutrition.Conclusion Advanced age(≥80 years),multiple medications,and malnutrition are independent risk factors for sarcopenia in elderly patients,while high BMI is a protective factor.关键词
