Abstract
Objective:To compare the therapeutic efficacy of holmium laser enucleation of the prostate(HoLEP)and bipolar plasma resection(TUPKP)in benign prostatic hyperplasia and the effect on sexual function.Method:164 cases of BPH patients diagnosed and treated in the Second People's Hospital of Liaocheng City from March 2023 to September 2024 were selected as the study subjects,and were divided into the HoLEP group and the TUPKP group by using the random number table method,with 82 cases in each group.Post-treatment indicators were compared between the two groups.Result:The operation time of HoLEP group was longer than that of TUPKP group,and the difference was statistically significant(P<0.05).Intraoperative bleeding,bladder irrigation time,and urethral retention time of HoLEP group were lower than that of TUPKP group,and the difference was statistically significant(P<0.05).Postoperatively,the maximum urinary flow rate of HoLEP group was higher than that of TUPKP group,and the residual urine volume was lower than that of TUPKP group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the International Prostate Symptom Score(IPSS)and Quality of Life(QOL)scores between the two groups(P>0.05).The International Index of Erectile Function-5(IIEF-5)score was lower in the TUPKP group than in the HoLEP group after treatment,and the difference was statistically significant(P<0.05).Serum total prostate-specific antigen(t-PSA)and free prostate-specific antigen(f-PSA)level were elevated in both groups after surgery,and the elevation was more obvious in the TUPKP group,and the difference was not statistically significant(P>0.05).The difference in the incidence of adverse reactions between the two groups was not statistically significant(P>0.05).Conclusion:Both HoLEP and TUPKP can achieve good efficacy and better safety in the treatment of BPH,HoLEP has less surgical bleeding,shorter bladder flushing time,shorter ureter retention time,lower patient stress level,and better postoperative sexual function.关键词
