超声引导下腹直肌鞘阻滞中不同剂量右美托咪定于腹腔镜胃癌根治术中的应用效果

Abstract
Objective:To investigate the effect of ultrasound-guided rectus abdominis sheath block with different doses of dexmedetomidine combined with ropivacaine in postoperative analgesia after laparoscopic radical gastric cancer surgery.Method:99 cases of patients who were admitted to the People's Hospital of Macheng City and proposed to undergo laparoscopic radical gastric cancer surgery from March 2022 to March 2024 were selected as the study subjects,and were divided into three groups of small,medium and large dose groups equally by simple randomization method,with 33 cases in each group.In all three groups,ultrasound-guided rectus abdominis sheath block was performed,and 0.2 μg/kg dexmedetomidine combined with ropivacaine was given to the small-dose group,0.5 μg/kg dexmedetomidine combined with ropivacaine was given to the medium-dose group,and 1.0 μg/kg dexmedetomidine combined with ropivacaine was given to the large-dose group.The sedation,analgesia,hemodynamic indices and adverse effects were compared among the three groups.Result:The Prince-Henry score was lower in the medium-dose group at T1,T2,and T3 than at the same moments in the low-dose and high-dose groups,respectively,whereas the Ramsay score was higher than at the same moments in the low-dose and high-dose groups,with a statistically significant difference(P<0.05).The mean arterial pressure(MAP)as well as heart rate(HR)at the t2 and t3 moments in the medium-dose group were lower than those at the same moments in the low-dose group and the high-dose group,and the difference was statistically significant(P<0.05).The difference in the incidence of adverse reactions among the three groups was not statistically significant(P>0.05).Conclusion:The application of medium-dose(0.5 μg/kg)dexmedetomidine combined with ropivacaine for ultrasound-guided rectus abdominis sheath block in laparoscopic radical surgery for gastric cancer has a good sedative effect,which is conducive to the improvement of hemodynamic indexes of the patients and the reduction of postoperative pain.关键词
