PEID与PETD治疗L5-S1椎间盘突出症的近远期疗效探究

Abstract
Objective:To investigate the short-and long-term efficacy of transforaminal endoscopic nucleus pulposus extraction(PETD)and interlaminar spinal endoscopic nucleus pulposus extraction(PEID)on L5-S1 intervertebral disc herniation.Method:80 patients with herniated intervertebral discs who underwent surgical treatment in the Second Hospital of Longyan City from June 2021 to December 2023 were selected as the study subjects,and they were divided into PEID and PETD groups according to the surgical methods,with 40 cases in each group.The clinical indexes of the two groups were compared,and the immediate efficacy[pain numeric rating scale(NRS)score,lumbar spine dysfunction index questionnaire(ODI)score]was compared between the two groups at 1 month postoperatively.The long-term efficacy(modified MacNab excellent rate)at 1 year after surgery was compared between the two groups,and the occurrence of complications in the two groups was counted.Result:Surgical time,bleeding,and number of fluoroscopies were lower in the PEID group than in the PETD group,and the difference was statistically significant(P<0.05).There was no statistically significant difference between the two groups in terms of time out of bed and postoperative hospitalization time(P>0.05).Preoperatively,there was no statistically significant difference in NRS and ODI scores between the two groups(P>0.05).One month after surgery,NRS decreased and ODI scores increased in both groups compared with before treatment,and the difference was statistically significant(P<0.05).The difference in NRS and ODI scores between the two groups was not statistically significant(P>0.05).At 1 year after surgery,the difference in MacNab excellence rate between the two groups was not statistically significant(P>0.05).There was no statistically significant difference in the comparison of complication rates between the two groups(P>0.05).Conclusion:PEID and PETD are comparable in treating L5-S1 disc herniation in terms of short-and long-term effects and complications.However,the duration of PEID is shorter,and it causes less blood loss.Moreover,the fluoroscopy frequency is lower.关键词
