抗肿瘤治疗相关心脏毒性是肿瘤幸存者面临的严峻挑战.基于《黄帝内经》"阳化气,阴成形"理论,本文系统探讨了抗肿瘤治疗相关心脏毒性的中医病因病机及辨治策略.化疗、放疗、靶向及免疫治疗等抗肿瘤手段,其"毒邪"可破坏心之气血阴阳动态平衡,核心病机可高度概括为"阳损气化失权,阴伤形质受损,浊毒内蕴为变",此三者相互胶结影响,共同构成了抗肿瘤治疗心脏毒性复杂多变的临床表现.据此核心病机,笔者团队确立"调和阴阳"为治疗总纲,并紧扣"阳损-阴伤-浊毒"的病理核心,提出以下辨治思路:温扶阳气,复气化之功;滋阴培元,固形质之基;化浊解毒,通心络之滞.

Cancer therapy-related cardiotoxicity poses a severe challenge for cancer survivors.Based on the theory of"yang transforming into qi,and yin constituting form"in Huangdi Neijing,this paper systematically explores the etiology,pathogenesis,and differentiation and treatment strategies of cancer therapy-related cardiotoxicity in traditional Chinese medicine.Anti-tumor modalities,including chemotherapy,radiotherapy,targeted therapy,and immunotherapy,induce"pathogenic toxins"that disrupt the dynamic balance of qi,blood,yin,and yang in heart.The core pathogenesis can be summarized as"yang impairment leading to dysfunctional qi transforming,yin injury resulting in structural damage,and accumulation of turbid toxins triggering pathological changes".These three mechanisms intertwine and collectively underlie the complex clinical manifestations of cancer therapy-related cardiotoxicity.Accordingly,the authors establish"harmonizing yin and yang"as the overarching therapeutic principle.Centered on the pathological core of"yang impairment-yin injury-turbid toxins",the following treatment approaches are proposed:warming and strengthening yang qi to restore transformation function,nourishing yin and fortifying the foundation to stabilize physical structure,and resolving turbid toxins to unblock heart collateral stagnation.
