从"阳郁阴结"论继发性淋巴水肿的因机治疗
Etiology,pathogenesis and treatment of secondary lymphedema from the perspective of"yang stagnation and yin binding"

Abstract
"Yang stagnation and yin binding"runs through the whole course of secondary lymphedema as its etiology and pathogenesis.In terms of its pathogenesis,yang deficiency due to insufficient qi transformation and yang stagnation due to disorders of qi movement are the causes of the disease at its initial stage.Dampness,phlegm and blood stasis constitute the pathogenic factors for the progression of secondary lymphedema."Stagnation"owing to"the interaction of deficient qi and blood and those pathogenic factors"features the core pathogenesis of advanced subcutaneous tissue fibrosis elephantiasis."Toxins"tend to lead to the development of complications in secondary lymphedema as they are susceptible to transition.In terms of its treatment,yin and yang are regulated and balanced,with"relieving yang stagnation and dispersing yin binding"as the core.Wind drugs are applied to open the sweat pores and drive the pathogens to the surface,dispersing stagnation and obstruction with pungent medicines,tonifying the liver and spleen,regulating qi and removing depression.Medicines such as suberect spatholobus stem,dutchmanspipe vine,kadsura pepper stem,orientvine vine are used to dredge collaterals,resolve stagnation and hard masses,activate blood circulation and remove blood stasis,benefit qi and promote blood circulation to break up masses and eliminate swelling,warmly disperse stagnation and warmly remove blockades in collaterals to soften hard masses and eliminate swelling.It is advised to combine internal and external treatment methods,and to modify treatment methods in accordance with specific syndromes.This study investigates secondary lymphedema from the perspective of"yang stagnation and yin binding",hoping to provide new ideas for the dynamic clinical understanding,prevention and treatment of this disease.关键词
