The Predictive Value of MRI Combined with Serum IL-1β and IL-17 for Sentinel Lymph Node Metastasis in Breast Cancer

Abstract
Objective To explore the predictive value of magnetic resonance imaging(MRI)combined with serum interleukin(IL)-1β and IL-17 for sentinel lymph node(SLN)metastasis in breast cancer.Methods We retrospectively selected 202 breast cancer patients admitted to Qingdao Central Hospital,University of Health and Rehabilitation Sciences from January 2022 to December 2024.The patients were divided into a metastasis group(53 cases)and a non-metastasis group(149 cases)based on the presence or absence of SLN metastasis.All patients underwent MRI examination upon admission,and the quantitative MRI parameters(long-to-short diameter ratio,signal enhancement ratio[SER],and apparent diffusion coefficient[ADC]value)were obtained.Serum IL-1β and IL-17 levels were detected using chemiluminescence immunoassay.Multivariate Logistic stepwise regression analysis was used to identify influencing factors for SLN metastasis in breast cancer.The ROC curve and AUC were used to evaluate the predictive value of MRI quantitative parameters,serum IL-1β,and IL-17 individually and in combination for SLN metastasis in breast cancer.Results The long-to-short diameter ratio,SER,and serum IL-1β and IL-17 levels in the metastasis group were higher than those in the non-metastasis group,while the ADC value was lower[(1.39±0.16)vs(1.03±0.12),(86.93±10.58)%vs(64.91±8.44)%,(14.76±4.35)pg/mL vs(5.09±1.28)pg/mL,(269.71±23.94)pg/mL vs(247.15±19.46)pg/mL,(0.81±0.17)×10-3 mm2/s vs(0.94±0.15)×10-3 mm2/s,P<0.05].Multivariate Logistic stepwise regression analysis showed that vascular invasion,long-to-short diameter ratio≥1.21,SER≥75.92%,IL-1β≥9.93 pg/mL,and IL-17≥258.43 pg/mL were risk factors for SLN metastasis in breast cancer,while ADC>0.88×10-3 mm2/s was a protective factor(OR=2.394,95%CI:1.464-3.916,P=0.001;OR=2.678,95%CI:1.680-4.269,P<0.001;OR=3.360,95%CI:1.885-5.991,P<0.001;OR=2.751,95%CI:1.637-4.625,P<0.001;OR=3.171,95%CI:1.857-5.414,P<0.001;OR=0.259,95%CI:0.150-0.449,P<0.001).The AUC for the combined detection of MRI quantitative parameters(long-to-short diameter ratio,SER,ADC)and serum IL-1β and IL-17 in predicting SLN metastasis was significantly superior to individual predictions(Z=8.124,11.807,9.518,10.843,12.415,P<0.001).Conclusion The MRI quantitative parameters(long-to-short diameter ratio,SER,ADC)and serum IL-1β and IL-17 levels are all associated with SLN metastasis in breast cancer and can serve as important indicators for predicting SLN metastasis.Moreover,the combined detection of MRI quantitative parameters with serum IL-1β and IL-17 demonstrates superior predictive value.关键词
