血清DBP、25-(OH)D水平对早发型子痫前期患者不良妊娠结局的预测价值
The Predictive Value of Serum DBP and 25-(OH)D Levels for Adverse Pregnancy Outcomes in Patients with Early-Onset Preeclampsia

Abstract
Objective To explore the predictive value of serum vitamin D binding protein(DBP)and 25-hydroxyvitamin D[25-(OH)D]for adverse pregnancy outcomes in patients with early-onset preeclampsia.Methods A total of 80 patients with early-onset preeclampsia admitted to Qinhuangdao First Hospital from February 2022 to February 2024 were included in the case group,and another 100 healthy pregnant women receiving antenatal care during the same period were included in the control group.Serum DBP and 25-(OH)D levels were detected by ELISA in all participants.Based on pregnancy outcomes,the 80 patients with early-onset preeclampsia were divided into a good outcome group(n=50)and a poor outcome group(n=30).The predictive value was analyzed using receiver operating characteristic(ROC)curves.Influencing factors were assessed by multivariate logistic stepwise regression analysis.Results Serum DBP and 25-(OH)D levels in the case group were significantly lower than those in the control group(P<0.05).Levels in the poor outcome group were significantly lower than those in the good outcome group(P<0.05).The area under the curve(AUC)(95%CI)for predicting pregnancy outcomes using serum DBP,25-(OH)D,and their combination were 0.731(0.686-0.781),0.829(0.784-0.879),and 0.901(0.856-0.951),respectively.The combined AUC was higher than that of either marker alone(Z=12.431,10.752;both P<0.05).The poor outcome group had higher proportions of 24-hour proteinuria≥2.0 g/24 h,D-dimer≥0.5 mg/L,and platelet count<100×109/L compared to the good outcome group(P<0.05).Multivariate analysis identified 24-hour proteinuria(OR=2.303,95%CI:1.215-4.362),D-dimer(OR=2.073,95%CI:1.042-4.125),serum DBP(OR=3.254,95%CI:1.906-5.557),and serum 25-(OH)D(OR=3.360,95%CI:1.852-6.097)as independent influencing factors for pregnancy outcomes(all P<0.05).Conclusion The serum DBP and 25-(OH)D levels are reduced in patients with early-onset preeclampsia,both of which have been confirmed as independent influencing factors for adverse pregnancy outcomes.The combined detection of the two is expected to become a new biological predictor for predicting pregnancy outcomes in such patients.关键词
