基于"随其所得而攻之"理论探讨慢性阻塞性肺疾病辨治思路
Discussing the treatment ideas of chronic obstructive pulmonary disease based on the theory of"attacking pathogen according to its location and characteristics"

Abstract
Chronic obstructive pulmonary disease(COPD)is one of the more difficult respiratory diseases to treat clinically,with a variety of clinical manifestations,including chest tightness,wheezing,coughing,coughing up sputum,and difficulty breathing.With an extremely high mortality rate and a long and complex treatment process,the disease has become a medical challenge that cannot be ignored.The pathogenesis of COPD has not been fully clarified by modern medicine.Traditional Chinese medicine believes that the etiology of the disease is based on the weakness of lung Qi(气),and the body feels external evils such as wind,water-dampness and blood stasis,which migrate to the five viscera,thus leading to the development of the disease.The pathologic changes and symptoms of COPD in the clinic are intrinsically related to the theory of"attacking pathogen according to its location and characteristics"in Jingui Yaolue(《金匮要略》).This theory embodies the connotation of"treating the disease at its root"in traditional Chinese medicine.The author draws on the wisdom of the ancients,from the three aspects of"attack invisible evil"and"attack and treat disease in the combined internal organs"and"medication should be administered according to the preferences of the five organs",in-depth understanding of the treatment principle of"attacking it according to its combination".Based on the theory of"attack invisible evil",the Xuhua Xiaoqinglong decoction(虚化小青龙汤),the Kuanxiong Lifei decoction(宽胸理肺汤),the Sanzi Yangqin decoction(三子养亲汤),and the Huatan Tongluo decoction(化痰通络汤)were used according to syndrome differentiation,and the therapeutic idea of using the Xiaochaihu decoction(小柴胡汤)to treat COPD with"heat enters the blood chamber"is put forward.For the treatment method of"attack and treat disease in the combined internal organs",the tonic and cathartic methods are appropriately used to care for the five organs,playing a balance of Yin(阴)and Yang(阳).When the condition of COPD is stabilized,according to the connotation of"medication should be administered according to the preferences of the five organs",the physiological and pathological characteristics of the lungs,the spleen,the kidneys and the heart are taken into account in the segmental treatment.Combining Chinese medicine with the principle of"attacking pathogen according to its location and characteristics"can effectively regulate and treat the symptoms of COPD,and further explore the therapeutic strategies for COPD.One clinical case is also attached to provide a reference for the management of COPD.关键词
