Abstract
Diabetic lower extremity vascular disease is one of the common complications of diabetes mellitus.The patient's blood vessel wall is in a long-term hyperglycemia state,which causes the deposition of advanced glycation end products and then damages the vascular endothelial cells,resulting in affected limbs cold,numbness,pain and other manifestations.Without timely treatment,there is a risk of severe ischaemia of the lower limbs such as foot ulcers and gangrene,or even amputation,which not only severely affects the life of patients,but also increases the risk of cardiovascular events and endangers the life of patients.Diabetic lower extremity vascular disease belongs to the category of diabetes Bi syndrome(痹证)and wilting disease in traditional Chinese medicine(TCM).Diabetes is mostly caused by spleen Yin(阴)depletion due to dietary irregularities,labour disorders,emotional disorders,physical weakness and strain,etc.Spleen Yin and fluid is depleted,dryness and heat are excessive,and the two interact with each other,which further aggravates the depletion of body fluid,resulting in body fluid loss and blood loss.Veins and tubes fail to be filled up,blood veins are obstructed due to stagnant blood flow,and blood stasis transforms into heat and injures Yin with a large amount of fluid loss.The volume of blood is decreased,which further aggravates the blood circulation stagnation,resulting in numbness and pain in both lower limbs,fear of cold and like warmth and other symptoms.YE Gui,a TCM master in Qing Dynasty,put forward the theory of"replenishing body fluids with sweet medicines"in Wenre Lun(《温热论》),which was extended by later TCM practitioners to the treatment of Yin fluid deficiency diseases with sweet medicines to protect spleen for restoring body fluid in stomach.In this study,taking Yin fluid deficiency as the core pathogenesis,starting from the theory of"replenishing body fluids with sweet medicines",the correlation between diabetic lower limb vascular lesions and"replenishing body fluids with sweet medicines"is explored.This disease is based on the disconnection of Qi(气)and blood,and Yin and Yang(阳)due to Yin deficiency of Yangming meridians(阳明经),accompanied by asthenic fire burning pulse wall caused by lung and stomach dryness-heat.Blood stasis changes,Yin fluid is burned,blood flow is not smooth,which causes blood stasis and pulse veins obstruction,resulting in blood vessel damage.In terms of the treatment of diabetic lower limb vasculopathy,the methods of benefiting Qi and nourishing Yin,clearing heat and moistening dryness,and activating blood circulation and removing blood stasis are emphasized to provide new ideas and methods for the clinical treatment of the disease.关键词
