Treating diabetic gastroparesis based on the theory of mutual interference of clear Qi and turbid Qi

Abstract
Diabetic gastroparesis is a pathological state of hyperglycemia accompanied by gastrointestinal dysfunction,which is a complication of diabetes.It often clinically manifests as postprandial fullness,upper abdominal discomfort,nausea,vomiting,etc.This disease not only delays the digestive function of the body,but also reduces the efficacy of diabetes drugs and the quality of life of patients.Deputy Chief Physician GAN Decheng is a famous doctor in Longshang area,inherits TCM ideas of his great-grandfather GAN Dingtian,grandfather GAN Huiting and father GAN Wenyuan,and has accumulated rich experience in the diagnosis and treatment of diabetes and endocrine diseases for more than 30 years.In terms of the treatment of diabetic gastroparesis,GAN Decheng has unique insights,and believes that diabetic gastroparesis is a syndrome of essence deficiency and manifestation excess,which is caused by Yang(阳)injury due to Yin(阴)injury after prolonged dissipation of diabetes,eventually causing dysfunction of spleen and stomach and the pathological state of abnormal Qi(气)circulation.The purpose of treatment is to remove ointment lipid and turbid phlegm to restore Qi circulation of spleen and stomach.The Gesang Huazhuo decoction(葛桑化浊汤)is made to invigorate Qi and strengthen spleen for ascending clear Qi and decending turbidity Qi.In the prescription,Gegen(Radix Puerariae Lobatae)and Sangye(Folium Mori)are sovereign medicines with ascending clear Qi function and descending turbidity Qi function respectively,making spleen Qi rising and healthy,and stomach Qi harmony.Huangqi(Radix Astragali),Shanyao(Rhizoma Dioscoreae),and Jineijin(Endothelium Corneum Gigeriae Galli)are used secondarily to supplement middle Qi,and harmony stomach to remove distention,which can strengthen spleen and stomach and make other viscera healthy.With the accelerated pace of modern life and the increasing pressure of people's life and work,gastrointestinal dysfunction has become a common phenomenon in patients with diabetes.Deputy Chief Physician GAN Decheng believes that the treatment of diabetic gastroparesis should not only focus on spleen and stomach,but also focus on the regulation of blood glucose level of patients,so as to reduce the tendency of Yin deficiency and dryness-heat caused by the excess of fat and sugar turbidity,then ameliorate Qi circulation disorder of spleen and stomach due to Yin deficiency for a long time,and prevent the digestive tract dysfunction caused by large fluctuations in blood glucose level.By the method of ascending clear Qi and descending turbidity Qi,Deputy Chief Physician GAN Decheng prepares the Gesang Huazhuo decoction for treating diabetic gastroparesis,and the clinical effect is significant,which prevents the pathological process of diabetic gastroparesis from the source.This study combines with one case to elaborate the clinical experience of Deputy Chief Physician GAN Decheng in treating diabetic gastroparesis experience,in order to provide more reference.关键词
