基于"络病理论"探讨免疫球蛋白A肾病的病机与治疗
The pathogenesis and treatment of IgA nephropathy based on the collateral disease theory
摘要
Abstract
IgA nephropathy is one of the most common primary glomerular diseases,with pathological characteristics of IgA deposition in the mesangial region of glomeruli.Its main clinical manifestations are asymptomatic microscopic hematuria or gross hematuria,which may be accompanied by proteinuria,edema and hypertension disease in different degrees.At present,the specific pathogenesis of IgA nephropathy is not fully understood,and there is no specific treatment plan.Hormones,immunosuppressants and symptomatic treatment are mainly used clinically,but most patients stop taking medicines due to into rance of adverse reactions.The treatment of IgA nephropathy is still a clinical difficult point.In recent years,traditional Chinese medicine has played an important role in the prevention and treatment of IgA nephropathy,and clinical studies have proved that the application of traditional Chinese medicine in the treatment of IgA nephropathy can delay the progression of the disease,ameliorate clinical symptoms,and reduce recurrence and complications.Based on the collateral disease theory in traditional Chinese medicine,combined with the research on kidney microscopic pathology in modern medicine,the etiology and pathogenesis of IgA nephropathy is analyzed by the author from the aspects of pathological factors such as pathogenic wind,damp-heat,and blood stasis.Furthermore,the treatment principles of dispelling wind and relieving exterior syndrome,clearing away heat and dampness,and strengthening Qi(气)and activating blood are summarized,among which dredging collaterals runs throughout the whole treatment process.One medical case of treating the spleen and kidney Qi deficiency plus blood stasis type IgA nephropathy by the method of strengthening Qi and activating blood to dredge collaterals is attached to provide more ideas for the clinical prevention and treatment of this disease.关键词
