Abstract
Objective:A network Meta-analysis was performed to evaluate the clinical efficacy and safety of different TCM methods in the treatment of hyperlipidemic acute pancreatitis.Methods:From CNKI,Wanfang,VIP,SinoMed,PubMed,EMBASE,Web of Science databases,randomized controlled trial studies from the inception of each database to September 1,2024 were retrieved by computer.The quality of the included literature was evaluated by the modified Jadad scale and Cochrane risk bias tool,and the relevant data was analyzed by Revman 5.4,STATA 18 and SPSS 27 software.Results:A total of 25 randomized controlled trial studies with a sample size of 1 950 were included finally,including 7 TCM treatment measures of the Dachaihu decoction(大柴胡汤),the Dachengqi decoction(大承气汤),the Qingyi decoction(清胰汤),the Chaishao Chengqi decoction(柴芍承气汤),the Danshen injection(丹参注射液),the Shuxuetong injection(疏血通注射液),and the Xuesaitong injection(血塞通注射液).The network Meta-analysis showed that in terms of improving the clinical effective rate,the intervention measure ranked first in the surface under the cumulative ranking curve(SUCRA)was the Danshen injection plus conventional Western medicine treatment[OR=7.68,95%CI(2.74,21.52)].In terms of reducing serum amylase level,the intervention measure ranked first in SUCRA was the Dachengqi decoction plus conventional Western medicine[WMD=-79.36,95%CI(-183.37,24.65)].In terms of shortening the recovery time of serum amylase level,the intervention measure ranked first in SUCRA was the Qingyi decoction plus conventional Western medicine[WMD=-1.90,95%CI(-2.92,0.88)].In terms of reducing serum TG levels,the intervention measure ranked first in SUCRA was the Qingyi decoction plus conventional Western medicine[WMD=-5.70,95%CI(-8.83,-2.58)].In terms of shortening the remission time of abdominal pain,the intervention measure ranked first in SUCRA was the Qingyi decoction plus conventional Western medicine[WMD=-3.58,95%CI(-5.25,-1.92)].In terms of reducing serum CRP level,the intervention measure ranked first in SUCRA was the Qingyi decoction plus conventional Western medicine[WMD=-45.13,95%CI(-66.52,-23.73)].In terms of reducing serum IL-6 level,the intervention measure ranked first in SUCRA was the Qingyi decoction plus conventional Western medicine[WMD=-54.93,95%CI(-66.72,-43.14)].In terms of reducing serum TNF-α level,the intervention measure ranked first in SUCRA was the Chaishao Chengqi decoction plus conventional Western medicine[WMD=-71.72,95%CI(-120.73,-22.71)].Conclusion:The results showed that the combination of TCM and Western medicine in the treatment of hyperlipidemia acute pancreatitis could improve the clinical effective rate,shorten the time of abdominal pain relief and the recovery of serum amylase level,and reduce the serum levels of amylase,TG,CRP,IL-6 and TNF-α,with fewer adverse reactions and reliable efficacy and safety.Limited by the sample size and quality of the included literature,this conclusion needs to be verified by more large-sample and high-quality randomized controlled trial studies.关键词
