The TCM connotation of the mechanism of pyroptosis and inflammatory oxidative stress of premature ovarian failure based on the NLRP3/CASP1/GSDMD signaling pathway

Abstract
For women,liver is congenital,and kidney is the origin of congenital.According to the theory of traditional Chinese medicine(TCM),premature ovarian failure(POF)is closely related to liver and kidney.There is ministerial fire in liver and kidney,which can produce the essence blood and warm the Zang-fu organs(脏腑).Clinical studies have suggested that"frenetic stirring of ministerial fire"is considered as an important link in the pathogenesis of POF in TCM.On the one hand,frenetic stirring of ministerial fire can seize the essence and damage Yin(阴),causing the"disharmony between heart and kidney,insufficient liver blood"of original deficiency;on the other hand,the blazing sovereign fire and ministerial fire induces liver wind,causing liver dysfunction,dysfunction of spleen in transportation due to liver stagnation invading spleen,and Qi(气)stagnation and phlegm obstruction,with not smooth blood flow.The pathogenic factors of"stagnation","phlegm",and"blood stasis"are in conflict with the blazing sovereign fire and ministerial fire,the pathogenic factor of"heat poison"is gradually formed and blocks the uterine collaterals,then forming excessive symptoms.Eventually,the pathological state of root deficiency and excessive symptom is formed,which promotes POF.In addition,modern studies believe that the pathogenesis of POF is closely related to inflammation and oxidative stress.Based on this,it is suggested that frenetic stirring of ministerial fire may also be the fundamental cause of triggering inflammation and oxidative stress to trigger POF.Inflammatory response and oxidative stress may lead to the release of inflammatory factors and the accumulation of toxic oxygen free radicals,thus activating the NLRP3 inflammasome to initiate the pyroptosis pathway and aggravating the process of inflammation response and oxidative stress,which can be regarded as the microscopic manifestation of the excessive symptom state of POF.The activation of GSDMD directly causes cell pyroptosis,which can be considered as the biological basis of root deficiency of POF caused by frenetic stirring of ministerial fire and the destruction of essence and Yin.Based on the theory of ministerial fire proposed by ZHU Zhengheng,the method of nourishing kidney and clearing heart,and resolving stagnation and removing phlegm can be considered as an important therapeutic principle of POF in TCM.The method is possible to achieve the therapeutic effect on POF by regulating the pathway of pyroptosis to ameliorate inflammatory response and oxidative stress,so as to reveal the connotation of the pathogenesis of POF caused by frenetic stirring of ministerial fire,and provide ideas for the TCM diagnosis and treatment of POF.关键词
