TCD/TCCD与DSA在评估颈动脉狭窄侧支开放中的互补关系研究

Abstract
Objective:To explore the advantages,disadvantages and complementary relationship between transcranial doppler(TCD)/transcranial color doppler(TCCD)and digital subtraction angiography(DSA)in assessing collateral opening in cases of severe stenosis or occlusion of the internal carotid artery.Methods:The DSA and TCD/TCCD examination reports of 63 patients with severe stenosis(≥70%)or occlusion of the internal carotid artery were analyzed.Using DSA as the gold standard,the sensitivity and accuracy of TCD/TCCD in detecting the opening of the leptomeningeal anastomosis branch of the anterior communicating artery(ACoA),the posterior communicating artery(PCoA),the soft meningeal anastomotic branches between posterior cerebral artery(PCA)and middle cerebral artery(MCA)were analyzed;and the differences between two tests for the opening of the ophthalmic artery(OA)were analysed.Results:The Kappa value for the consistency test between TCD/TCCD and DSA in detecting ACoA opening was 1(P＜0.001);TCD/TCCD and DSA were consistent in detecting the opening of anterior and posterior communicating arteries;for PCoA opening,the Kappa value was 0.888(P＜0.001);for OA opening,it was 0.653(P＜0.001);and for the soft meningeal anastomotic branches between PCA and MCA,the Kappa value was 0.777(P＜0.05).TCD/TCCD was more sensitive and efficient than DSA in detecting OA opening,but significantly inferior to DSA in detecting the opening of the soft meningeal anastomotic branches.Conclusion:In cases of severe stenosis or occlusion of the internal carotid artery,TCD/TCCD can assist DSA in evaluating collateral opening and has the potential for wider application.关键词
